Registered Nurse (0.5 FTE) – MVP Clinic

Organization Description The Barrie & Community Family Health Team (BCFHT) is one of the largest family health teams in Ontario. We are comprised of Physicians, Registered Nurses, Nurse Practitioners, Dietitians, Pharmacists, Diabetes and Respiratory Educators, Mental Health Counsellors and an Administrative team. Our team is focused on improving access to comprehensive, patient-centred, team-based primary health care, which supports self management, emphasizes health promotion and disease prevention, and enhances the management of individuals with chronic diseases through programs that are well linked with other local and community services. Position Description Reporting to the Clinical Manager, the RNs work with an interdisciplinary team providing patient centered care for the MVP Clinic and occasional support to other BCFHT clinics.  The MVP Clinic serves patients with multiple complex medical conditions who may not be attached to a primary care physician.  This is a challenging position ideal for someone who enjoys complex populations and who is looking to maintain strong clinical and communication skills. Primary Responsibilities
  • Triage incoming referrals
  • Provide professional and patient centered nursing care in accordance with regulatory body standards and guidelines
  • Perform patient assessments and engage patients in the development and revision of care plans
  • Provide health teaching
  • Collaborate with clinical team members and social supports and be a strong patient advocate
  • Clearly and accurately document clinical interactions within the EMR in accordance with CNO standards
  • Participate in clinical case conferences and communicate appropriate patient information within the circle of care
  • Maintain competency in nursing skills
  • Attend meetings with community partners and stakeholders as indicated
Education, Qualifications and Skills
  • Registration in good standing with the College of Nurses of Ontario
  • BScN or equivalent combination of education and experience
  • Current basic cardiac life support
  • 3-5 years of nursing practice with complex populations - experience in a variety of settings an asset (such as ICU, emergency, outpatient, public health, primary care)
  • Demonstrated experience in Mental Health and/or addictions
  • Ability to maintain information in confidence
  • Possess strong communication, critical thinking, clinical knowledge, time management and patient engagement skills
  • Experience working with electronic medical records systems considered an asset
  • Working knowledge of Microsoft Office (Word/Excel/Outlook)
