Company Name: Two Rivers Family Health Team Position Title: Registered Dietitian/Certified Diabetes Educator Term: Full time Location: Two Rivers Family Health Team, Cambridge, Ontario Deadline for application: June 20, 2014 Description Two Rivers Family Health Team is looking for a dynamic Registered Dietitian, with experience working with diabetes in a clinical or outpatient setting, to work with 21 family physicians and other health care practitioners in the Diabetes Education Program. Two Rivers is an inter-professional health care team providing excellent primary health care in a respectable, mutually accountable work environment. Two Rivers FHT has a partnership with Pace Consulting Group. Pace Consulting Group is a wholly owned subsidiary of We Care Home Health Services and CBI Health Group. Pace delivers a diverse range of health care services, including physiotherapy, occupational therapy, registered massage therapy, speech-language pathology, dietetics, social work, and mental health services in EAP and corporate counseling services and primary health care fields, such as Family Health Teams. Duties include, but are not limited to:
  • Providing outpatient diabetes nutrition counseling services, which include conducting nutrition assessments, developing nutrition care plans and providing regular follow up care
  • Providing group education
  • Participating in ongoing development and initiatives of the Diabetes Education Program
  • Working collaboratively with physicians and other inter-disciplinary health professionals
Qualifications The ideal candidate should:
  • Be a Registered Dietitian, and member in good standing with the College of Dietitians of Ontario, and possess professional liability insurance
  • Have a minimum of 3 years’ experience working with diabetes in a clinical or community setting
  • Be able to identify nutrition problems, assess nutritional status of patients, develop nutrition care plans and evaluate and monitor effectiveness of nutrition interventions in relation to the management of diabetes
  • Possess excellent nutrition counseling skills, as well as oral and written communication skills
  • Have experience in providing group education
  • Be able to work collaboratively within an interdisciplinary health care team, but also be self-directed and able to work independently
  • Demonstrate a high level of competency with computers and information technology
  • Possess Certified Diabetes Educator certification, or working towards certification
