Certified Diabetes Educator (Full-Time/Temporary-1 Year Position)

In the North York Family Health Team (NYFHT), Physicians work with other healthcare professionals, such as Nurse Practitioners, Registered Nurses, Dietitians, Social Workers and Pharmacists to provide enhanced care to patients of the North York area.  Family Health Teams are a key piece of the Government of Ontario’s plan to build a healthcare system that promotes good health, reduces wait times and improves access to care. Position Information: This is a 1-year contract. The Registered Dietitian will provide specialty services in the delivery of medical nutrition therapy. The RD will  work with our interdisciplinary family health team by collecting data, assessing, monitoring and reassessing patients in a primary care setting, ensuring that patients receive appropriate and optimal nutrition care.  The RD will also counsel individuals and groups within our centralized Diabetes Education Program. The RD will identify priorities, strategies and interventions in order to contribute to patients’ individualized treatment programs.  The RD will help patients manage their chronic illness through nutrition interventions.  In addition, the RD will coordinate and lead group education sessions on various nutrition topics.  The RD will participate in professional development and educational activities, including providing clinical education to interns and other healthcare professionals.  Some evening hours and travel between sites will be required. Qualifications:
  • Bachelor’s degree in Food and Nutrition or equivalent degree with an accredited dietetic internship or equivalent supervised practical training required, post-graduate education preferred
  • A member in good standing with the College of Dietitians of Ontario
  • Current registration as a Certified Diabetes Educator
  • Previous experience in Diabetes Education
  • Skilled in identifying nutrition problems, assessing nutritional status of patients, developing nutrition care plans and evaluating and monitoring the effectiveness of nutrition interventions
  • Demonstrated experience in individual and group facilitation
  • Previous experience managing chronic diseases through nutrition interventions
  • Demonstrated computer literacy is required to facilitate the use of electronic medical records
  • Demonstrated ability in problem solving using sound judgment and critical thinking
  • Strong organizational, interpersonal and communication skills
  • Ability to work independently and cooperatively in a busy multidisciplinary team
