Registered Dietitian (0.2 FTE) - Woodbridge Medical Centre FHT

Woodbridge Medical Centre FHT is situated in a vibrant, multi-cultural neighbourhood. We work within the community to address its health-related needs and are dedicated to providing a full range of high quality medical care.  WMC FHT aims to provide efficient, high quality health care services in the community with a focus on improving patient   access, health promotion and chronic disease management.  Our services are to be delivered with exceptional customer service while maintaining a healthy environment for our staff and patients. We are currently seeking a dynamic, innovative Registered Dietitian (0.2 FTE) to join our team. The Registered Dietitan will play an integral role in the care of the patients, and assist the FHT in achieving its vision and mission. The Registered Dietitian (RD) will support patient care and the delivery of programs, including healthy lifestyles, chronic disease and geriatric care. The dietitian will also provide physicians and Team providers with nutritional consultation, education and information that contribute to the health and wellness of patients and the knowledge and expertise of providers. Responsibilities
  • Works collaboratively with other team members to manage the needs of patients
  • Develops nutritional plans and provides nutritional counselling for patients based on comprehensive needs assessments
  • Promotes behaviour change related to food choices, eating behaviour and preparation methods to optimize health
  • Develop programs or workshops around identified nutrition issues at times and locations appropriate for patients
  • Participates as a clinical team member in the development and delivery of targeted programs, including, but not limited to, chronic disease, mental health and addictions, and Elder Care etc.
  • University degree and 3-5 years of relevant experience
  • Registration with the College of Dietitians of Ontario and proof of liability insurance
  • Thorough knowledge of diet and nutrition in a primary care context and the ability to function effectively in a team environment with physicians, other allied health providers and administrative staff
  • High level of interpersonal skills to deal with patients and their families in a holistic, compassionate fashion
  •  Knowledge of client-centred philosophy, change management, basic principles and practices of community service delivery, short-term treatment options and chronic disease management models
  • Knowledge of community resources connected to the provision of health promotion and illness prevention related services
  • Demonstrated skills in assessment, program planning, implementation and evaluation
