Primary Health Care Nurse Practitioner - Health for All FHT (Markham)

Overview Health for All Family Health Team is currently recruiting a Nurse Practitioner to cover a 12 month maternity leave.  The NP-PHC provides a range of independent and collaborative advanced nursing services with a focus on health promotion as well as the assessment and treatment of acute and chronic primary health conditions while working with our family physicians and inter-professional healthcare providers. Reporting Professional guidance and clinical support in matters concerning the provision of direct patient care will be provided by consultation with our family physicians.  The NP-PHC reports to the Executive Director for day to day operations. Duties & Responsibilities Clinical Responsibilities
  • Assess holistically and provide services to patients in all developmental stages
  • Complete health assessments, including a health history and physical examination.
  • Determine the need for and implement health promotion and disease prevention counselling.
  • Provide patient education through individual or group sessions to help in the understanding and management of particular health issues.
  • Formulate and communicate medical diagnoses.
  • Determine the need for and order the necessary screening and diagnostic laboratory tests and interpret results.
  • Prescribe drugs – with the exception of controlled substances and sedatives.
  • Participate in other activities as requested in collaboration with the inter-professional team.
Education/Staff Development
  • Support the learning and professional development of Family Medicine residents, medical students and inter-professional team members when appropriate and requested.
  • Participate in ongoing quality improvement and research activities related to the services delivered at Health for All.
Qualifications & Experience
  • Current and valid registration and liability coverage as a Registered Nurse in the Extended Class with the Registered Nurses Association of Ontario (RNAO) is required.
  • Master’s degree in nursing
  • Certification as a Primary Health Care Nurse Practitioner
  • 3 – 5 years’ experience as a Nurse Practitioner in a primary health care setting or a combination of hospital, community and/or public health experience
  • Strong clinical skills, eexperience and proficiency in current physical assessment and treatment methods within the NP scope of practice in primary health care
  • Nursing experience working with priority populations and working within a multi-disciplinary setting
  • Competency in computer based programs and skill and experience with Electronic Medical Records (Practice Solutions preferred)
  • Excellent oral and written skills
  • Ability to work with all members of the health care team, including physicians
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment
