Primary Care Registered Nurse - North York FHT (Toronto)

ORGANIZATION BACKGROUND In the North York Family Health Team (NYFHT), you will work with physicians and other healthcare professionals to provide comprehensive care within an inter-professional patient-centred care model.  .  Family Health Teams are a key piece of the Government of Ontario's plan to build a healthcare system that promotes good health, reduces wait times and improves access to care. POSITION SUMMARY The Primary Care Registered Nurse (PCRN)will provide comprehensive nursing services to the patient population of a family physician(s) practice clinic within the North York Family Health Team. The PCRN will assess, manage and support patients, providing appropriate documentation, and will work collaboratively with the physicians and multi-disciplinary team to develop and implement care plans and provide follow-up with the patient. The PCRN will work to their full scope of practice within the primary care setting; some duties will vary depending on the Clinic  and patient population. The PCRN will work with North York Family Health Team Physicians in the community, other health professionals and local and regional programming in a multi-disciplinary environment to ensure a high quality of evidence based health care to the practice population.   • To work collaboratively within a family practice environment to enhance the delivery of primary care services.
  • To receive patients and provide individualized disease prevention, health promotion and other appropriate services based on nursing knowledge and skills.
  • According to practice protocols, conduct assessments, initiate and implement management strategies and provide appropriate nursing interventions and follow-up.
  • To assist in the implementation of quality improvement strategies and possibly research activities to improve the quality and provision of primary care.
  • To participate in community needs assessment and program development, where and when required.
  • To educate, communicate, advocate, coordinate/navigate, and support patients as needed
Clinical Role
  • To work in conjunction with the physicians or independently as appropriate to incorporate the nursing process to provide patient centered, goal oriented care.
  • To develop long-term, trusting relationships with both patients and peers in providing primary care services.
  • To assist in the delivery of disease screening, management and health promotion programs according to clinical practice guidelines.
  • To participate in prenatal, perinatal, infant, child and elder care – previous experience is preferred
  • To perform immunizations and other injections, venipuncture, and ECGs as required.
  • To ensure the maintenance of accurate records in line with practice policy.
  • To act as a resource to other multidisciplinary team members, physicians, and community agencies where and when necessary.
  • To offer training support for other team members and assist in their professional development.

  • Be responsible for their own professional development through a variety of strategies highlighted in their personal development plan.
QUALIFICATIONS
  • Member in good standing with the College of Nurses of Ontario
  • Baccalaureate degree in Nursing preferred
  • Preference given to those with at least two years of nursing experience in a primary care
  • Practical experience in diabetes management (counselling and assessment) – Diabetes
  • Demonstrated computer literacy
  • Experience with electronic medical records preferred
  • Current CPR — Healthcare Provider
  • Must have own vehicle and valid driver’s license
  • Knowledge of and ability to use electronic medical records setting
  • Educator Certificate preferred
  • Demonstrated computer literacy
