Primary Care Registered Nurse - North York FHT (North York)

Primary Care Registered Nurse– 1 FTE Full-Time Permanent   Reporting To:  Clinical Manager                                                                                     Position Summary:   The Primary Care Registered Nurse (RNPC), under the direction of the Clinical Manager, will provide comprehensive nursing services to the patient population of a family physician(s) clinic within the North York Family Health Team. The RNPC will work to their full scope of practice within the primary care setting; working collaboratively with physicians and allied health professionals to optimize quality, evidence-based patient care.   Main Responsibilities and Position Objectives:  
  • Deliver quality primary nursing care to patients and their family, collaborating with other members of the multidisciplinary team in a family practice environment.
  • Conduct assessments, initiate and implement management strategies, provide appropriate nursing interventions and follow-up.
  • Deliver preventative care screening, chronic disease management, patient education and health promotion according to clinical practice guidelines.
  • Care for patients throughout the lifespan; delivering prenatal, perinatal, infant, child and elder care.
  • Perform immunization updates and other injections as required.
  • Develop long-term, trusting relationships with patients and families, utilizing a therapeutic patient-centered approach.
  • Act as a resource to other multidisciplinary team members, physicians, and community agencies.
  • Offer training support for other team members and assist in their professional development.
  • Participate in the implementation of quality improvement strategies, research activities, and/or program development as aligned with strategic clinical care priorities of the organization.
  • Partake in professional development and continuous learning.
  The Primary Care Registered Nurse should:
  • Have the ability to work independently and prioritize workload.
  • Possess strong teamwork skills.
  • Have excellent problem solving and critical thinking skills.
  • Have excellent written and verbal communication skills.
  Education & Experience:  
  • BScN required. Member in good standing with the College of Nurses of Ontario.
  • Preference given to those with at least two years of primary care experience.
  • Experience in diabetes management (counselling and assessment) – Diabetes
educator certification preferred.
  • Demonstrated computer literacy and experience with EMRs.
  • Current CPR— HCP certification.
  • Must have own vehicle and valid driver’s license.
