Physician Assistant – South East Toronto FHT

South East Toronto Family Health Team The South East Toronto Family Health Team (SETFHT) is an Interprofessional Family Health Team (FHT) undergoing exciting development and transformation in ensuring access to quality primary health care to our patients. This academic FHT is affiliated with the University of Toronto and is comprised of a team that includes physicians, medical residents, nurse practitioners, registered nurses, dietitians, social workers, mental health/addictions counsellor, care navigator and chiropodist, all supported by an exceptional administrative and clinical support team. Position Profile As a key member of the South East Toronto Family Health Team (SETFHT), and an integral part of an Interprofessional health care team, the Physician Assistant (PA) will deliver care in accordance with SETFHT policies, professional standards and medical directives.  You will perform routine clinical duties, and ensure that the clinical needs of the SETFHT patients and physicians are met. Reports to The Physician Assistant (PA) reports to, and is responsible to, the SETFHT Executive Director. All acts are conducted under the direction and supervision of a registered Physician, and in accordance with the College of Physicians and Surgeons of Ontario’s policy on delegation. The supervising physician is responsible for the medical care provided to the patient by the PA and can only delegate medical acts that are within their own scope of clinical practice. Authority The PA does not direct the activities of staff or a function without direction of FHT decision makers, more specifically the supervising physician. Decision Making The PA is not an autonomous health care provider, and cannot act as the principal medical decision maker. Position Summary While working with this team of health professionals in an Interprofessional collaborative primary care setting and under the supervision of a physician, your role will be split between two areas of care: primary clinical care and initiatives supporting our complex patient programs. For primary clinical care, the duties include, but are not limited to: Clinical Duties
  • Triage incoming patient calls. Assess patient health care needs, discuss test results and respond to questions pertaining to health promotion and disease prevention. Redirect calls to booking when necessary.
    • For patient appointments, conduct patient interviews and take medical histories, perform physical examinations and interpret findings;
    • Maintain the medical record alert system to recall patients for health promotion, disease prevention follow-up.
    • Maintain the medical record Health Maintenance Plans to ensure patient compliance to timelines for health promotion, disease prevention programs such as pap smears, mammograms, FOB testing, bone density, and diabetes care.
    • Administer and record immunizations and allergy injections.
    • Administer clinical procedures such as suture removal, skin and wound dressings, TB testing, wart treatment, and ear-syringing.
    • Perform blood pressure follow-ups, assessing response to medications, educating patients and promoting health through blood pressure management.
    • Complete pre-operative and MTO physical forms as far as possible.
    • Maintain accurate, concise and confidential documentation pertaining to patient history and encounter data.
Chronic Disease Management
  • Work collaboratively with the health care team in the development, implementation and monitoring of chronic disease management programs around key health issues such as cardiovascular disease, diabetes and obesity.
Interprofessional Collaboration
  • Develop and hone collaborative community relationships to facilitate patient access to services upon physician/health care team request.
  • Communicate ideas or concerns to the health care team. Participate in problem solving to enhance patient care.
  • Assist in the training and development of new staff.
Complex Patient Care:  Virtual Ward and Home Visit Program This will involve working as a patient case manager.  Working with a primary physician to ensure that complex patients are being assisted in all areas of life as necessary. Duties include, but are not limited to:
  • Frequent phone calls to patients, ensuring close follow-up care and education;
  • Home visits to house bound patients;
  • Communication across sectors (e.g. CCAC, TEGH, caregivers, Woodgreen etc…);
  • Arranging specialist appointments and ensuring transportation;
  • Clinic visits for virtual ward patients as above.
Performing other duties within your competencies as a physician assistant and certain acts under medical directives delegated to you by a physician. Education and Qualifications Completion of an accredited PA program or equivalent:
  • Graduate of an Accredited Physician Assistant Program.
  • Certified or eligible to be certified through the Physician Assistant Certification Council of Canada (PACCC) or the NCCPA (National Commission on Certification of Physician Assistants) for American educated/trained PAs.
  • Current BCLS and CPR certification preferred.
  • Ontario G driver’s license (car not required but may be easier for home visits).
Other Skills/Abilities
  • Excellent oral/written skills.
  • Ability to work effectively with all members of the health care team.
  • Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery.
  • Ability to develop strategies that successfully meet the needs of diverse patient, family and provider population.
  • Proficiency in the use of computer hardware and software (especially Practice Solutions).
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment.
Compensation is based on experience and will range from $37.00 to $38.49/per hour. Position also includes enrolment in the Healthcare of Ontario Pension Plan and benefits plan.
