Physician Assistant - Prime Care FHT (Milton)

Prime Care Family Health Team  The Prime Care Family Health Team is looking to recruit a dedicated and experienced Physician Assistant to our dynamic team.  The Physician Assistant will collaborate with our 14 Family Physicians, as well as work within a centralized location with our team members, including: Nurse Practitioners, Social Workers, Dietitian, Psychologist, Registered Nurses, Respiratory Therapist, etc. We are located in Milton; a thriving mixed urban and rural community of 100,000 in the Region of Halton, less than 1 hour from Toronto, Mississauga and Hamilton.   • Integrates Physician Assistant clinical skills, procedural skills and professional attitudes, which are directed to effective patient-centered care
  • Collect and interpret information, make appropriate clinical decisions, and carry out diagnostic and therapeutic interventions
  • Conducting patient interviews and taking medical histories
  • Performing physical examinations and interpret findings
  • Performing certain acts under medical directives delegated by a physician
  • Counselling patients on preventive healthcare
  • Track patients with one or more risk factors and notify for re-screening and assessment.
  • Organizing follow-up
  • Performing other duties within your competencies as a physician assistant.
  • Prepare Quarterly Service Reports for MOHLTC requirements
  • Attend monthly team meetings
Assessment Treatment/Management
  • Function effectively as a physician extender to provide optimal, ethical and patient-centered medical care
  • Apply clinical knowledge, appropriate to patient care
  • Perform a complete and appropriate assessment of a patient and formulate a clinical treatment plan
  • Implement effective management plans that include preventive and therapeutic interventions
  • Demonstrate appropriate procedural skills, both diagnostic and therapeutic
  • Seek appropriate consultation from the supervising physician and other health
 Education/Advocacy
  • Appropriately adapt scope of practice within the specific clinical setting of the supervising physician
  • Identify and appropriately respond to relevant ethical issues arising in patient care
  • Effectively and appropriately prioritize professional duties when faced with multiple patients and problems
  • Demonstrate compassionate and patient-centered care
  • Recognize and respond to the ethical dimensions in medical decision-making
Referrals/Collaboration
  • Receive referrals from FHT Physicians and FHT healthcare practitioners and consult with these referring practitioners
  • Provides consultation to health care team regarding clinical situations (within area of expertise)
  • Refer to IHP’s, as required
