Job Description:


  • Educates and counsels family members and caregivers regarding the impact of disability, injury or disease on the individual and their potential role within the recovery process
  • Educates and counsels to promote function and independence including health promotion and injury prevention
  • Assesses physical, emotional, and cognitive functioning with environmental considerations
  • Provides evaluation and follow-up services for individuals of all ages who have sustained traumatic injuries resulting in cognitive, physical, behavioral and/or psychological challenges
  • Evaluates cognitive and/or perceptual abilities
  • Conducts clinical observations of gross and fine motor, visual-perceptual-motor, and in-hand manipulation
  • Conducts mobility assessments and Assistive Device Program assessments
  • Evaluates the home, and work to assess the need for specialized equipment modifications and/or supports
  • Provides individualized treatment plans to develop, maintain, or augment function using evidence-based treatment modalities
  • Implements treatment plans and programs including but not limited to home modifications, mobility devices, falls prevention, caregiver support and education
  • Prescribes specialized adaptive equipment and teaches proper usage
  • Collaborates with other health care professionals and community service providers to promote comprehensive and coordinated care
  • Shares results of assessment and intervention with referring physicians and other Team clinicians as requested within a specified timeframe
  • Recommends and arranges for specialist referrals and appointments in consultation with physician(s)
  • Facilitates referrals to other services as well as communication and networking between the primary care practice and hospital or home care
  • Maintains relationships with networks, related professional organizations and other related service providers
  • Exercises reasonable care and caution in protecting confidential and sensitive information related to clients and personnel
  • Participates as a clinical team member in the development and delivery of targeted programs
  • Actively participates in clinical programs and services (including occasional evenings and weekends requirements)
  • Participates in the development, planning, and evaluation of the programs and services of the FHT
  • Undertakes home visits when required
  • Promotes awareness of the FHT services and programs to patients and community residents alike
  • Must be willing to travel to all CKFHT sites, when required

Qualifications, Skills and Experience

  • Completion of appropriate academic preparation (e.g. B.Sc.)
  • Must be ADP Certified
  • Membership with The Canadian Association of Occupational Therapists and evidence of liability insurance
  • Registration with the College of Occupational Therapists of Ontario
  • Knowledge and proficiency in current, evidenced-based methods and practices
  • Experience in program development, implementation, monitoring and evaluation
  • Additional training or experience in chronic disease management and geriatrics is considered an asset
  • Outstanding problem-solving and conflict resolution skills
  • Understanding of community resources
  • Knowledge and understanding of Continuous Quality Improvement
  • Strong problem-solving, decision-making and time management skills
  • Excellent verbal and written communication skills
  • Ability to work within a team or independently
  • Proficient in the use of the computer hardware and software
  • Prior experience and knowledge of electronic medical records
  • Willingness to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
  • Demonstrated understanding of the goals, structures, policies and procedures of the FHT
