Nurse Practitioner - Woodbridge Medical Centre FHT

Position: NURSE PRACTITIONER (NP) Status: Full Time – 1 Vacancy Sector: Community – Family Health Team (LHIN 8-Central) Compensation: $79,054.00 to $89,203.00 Deadline: June 30, 2013 Anticipated Start Date:  August 2014
Position Summary: Operating within his/her full scope of practice, the NP will function autonomously and collaboratively with patients, physicians, and the interdisciplinary team to support the Vision, Mission, and Core Values of WMCFHT and to ensure that organizational accountability systems are implemented.  The NP will support patient care and the delivery of programs including chronic disease and health promotion, by focusing on the identification, provision and management and will also provide comprehensive primary care both within a solo and team-based dynamic. Key highlights of the role include the management of patients, addressing patient outcomes specifically and globally, patient safety and quality of care delivery. The NP will have and demonstrate the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals and perform procedures within his/her legislated scope of practice. NP core competencies include expert and advanced technical skills and judgments relating to the primary care patient population. Responsibilities:
  • Participates in the development, planning and evaluation of treatment education, counseling and health promotion programs and activities
  • Assesses holistically and provides services to patients in all developmental stages, and to families and communities
  • Completes health assessments, including a health history and physical examination
  • Determines the need for, and orders from, an approved list of screening and diagnostic laboratory tests and interprets the results
  • Determines the need for, and orders and interprets reports of X-rays, ECGs and diagnostic ultrasounds for diagnosis
  • Monitors the ongoing therapy of patients with chronic stable illness by providing effective pharmacological, complementary or counseling interventions
  • Prescribes drugs from an approved list
  • Uses nursing strategies arising from the best available evidence and consistently incorporates the patient’s perspectives in care
  • Consults with a physician in accordance with the standards for consultation with physicians, and/or refers the patient to another health care professional
  • Recommends and arranges for specialist referrals and appointments in consultation with physician(s)
  • Facilitates referrals to other services as well as communication and networking between the primary care practice and hospital or home care
  • Maintains relationships with networks, related professional organizations and other related service providers
  • Uses community assessment data to identify community needs and resources and develops, implements and evaluates appropriate programming
  • Participates in health promotion and disease prevention counseling
  • Provide primary health care services within the scope of the RN-EC including:  physical assessments, treatment, diagnosis and interpretation of test results; therapeutics; health promotion/education/counselling; history taking, screening, referral, and follow up. Consult with physicians and/or other health care providers when patient's condition requires care beyond the scope of practice (RN-EC).
  • Supervise Nurse Practitioner students by providing orientation, setting up schedules, developing/implement training/learning plans and conducting individual teaching with shared patient contact.
  • Provide services during regular and extended hours on a scheduled basis, on a walk-in basis and on urgent basis
  • Work within a health care framework which emphasizes health promotion and education, disease prevention, and recognizes the special risk factors (social, physical, psychological etc) which may affect the health of patients.
  • Work in partnership with other members of primary care to share responsibility for planning, provision and coordination of patient care.
  • Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records, participating in chart reviews and discussions of problem cases and answering patient inquiries and directing them to the appropriate treatment/resource.
  • Participate in administrative activities by: providing input into program development, delivery and evaluation; collecting statistical data as required by WMC FHT; identifying areas of development with regards to policies, procedure and protocols; participating in staff, team and other committees; liaising with other institutions, organizations and health and social service professionals.
  • Chair of Quality Improvement Committee and Co-Chair of Patient Advisory Committee
Qualifications, Skills and Experience
  • Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred
  • Current Certificate of Registration with the College of Nurses of Ontario as an RN (EC) required
  • Current Membership with Registered Nurses Association of Ontario (RNAO)
  • Current Membership with Nurse Practitioners’ Association of Ontario (NPAO)
  • Evidence of liability insurance
  • Two years clinical experience in family medicine or a relevant ambulatory care environment
  • Demonstrated leadership and effective interpersonal skills
  • Demonstrated active participation in knowledge transfer
  • Experience and proficiency in current nursing physical assessment and treatment methods within the scope of practice of the Registered Nurse, Extended Class.
  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion, risk reduction and chronic disease management
  • Knowledge of client-centred philosophy, change management, basic principles and practices of community service delivery, short-term treatment options and chronic disease management models
  • Knowledge of community resources connected to the provision of health promotion and illness prevention related services
  • Works effectively, both independently and as part of an inter-disciplinary team
  • Possesses superior leadership, organization, research, evaluation, time management, communication and interpersonal skills
  • Exhibits a desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
  • Exhibits an ability to be open and non-judgmental
  • Ability to work flexible hours (some evening and weekend work may be required)
  • Knowledge of and proficiency in the use of computer systems, software, particularly in Microsoft Office (Word, Excel, Outlook) and knowledge and experience in the application of electronic medical records (EMR)
