Nurse Practitioner - Summerville FHT (Mississauga)

Position: Nurse Practitioner / Full-Time 37.5 hours per week / 5 Months Contract File #: FHT02-2014 Dept/Health System: Summerville Family Health Team (FHT) Reports to: Executive Director of the Health Team
Position Summary:  Operating within their full scope of practice, the Nurse Practitioner will work collaboratively with all members of the Family Health Team in the treatment of identified illnesses, promotion of patient health, prevention of disease. Job Responsibilities:
  • Conduct period examinations and risk assessments that may include the care of at risk populations, wellness exams, pap tests and breast exams, prenatal care, well baby visits and immunizations
  • Provide care to patients with specific intermittent illnesses as per FHT Program guidelines
  • Collaborate with other multi-disciplinary team members  and external partners as per FHT Program guidelines
  • Develop and conduct a broad range of health promotion and chronic disease management processes, programs, and patient education (group and individual) in collaboration with multidisciplinary team members
  • Provide Nursing input for the development of the FHT’s Program planning
  • Maintain accurate, concise and confidential EMR documentation pertaining to patient history and encounter data
  • Complete necessary FHT Program business documentation
  • Communicate ideas and concerns to the team and participate in problem solving
  • Assist in the training and development of new staff
  • Participate in educational sessions held by other FHT multi-disciplinary team members
  • Teach and mentor others on the role of Nurse Practitioner / within the Family Health Team
Key Qualifications:
  • 3 years working experience in a primary care setting is required
  • Current registration with the College of Nurses of Ontario as a Nurse Practitioner – Primary Health Care
  • Excellent health assessment/clinical skills
  • Health promotion skills including population health and stress management an asset
  • Certified  Diabetic Educator (CDE) certification an asset
  • CPR certification required
  • Knowledge of and working experience with computer systems and applications
  • Excellent verbal and written communication skills
  • Ability to  work collaboratively as a member of the FHT’s multi-disciplinary team
