Position: Nurse Practitioner Status: Contract 12-18 months Date: September 21, 2013 Reporting To: Executive Director and Peer Lead Salary: $80,000 to 89,203 yearly (37.5 hrs/wk) Supervision: Yes PURPOSE The Nurse Practitioner (NP) is an autonomous and community based practitioner providing primary care. The NP will work with diverse populations in a family practice setting and/or a variety of contexts and practice settings. The NP will work in collaboration with the family physicians and the interdisciplinary team. The NP is a resource to clients, other nurses and health care professionals. The NP will provide leadership in advocating for the patient. DUTIES AND RESPONSIBILITIES 1. Works autonomously and collaboratively. 2. Diagnoses and treats ongoing or new medical conditions. 3. Monitors ongoing care, orders appropriate screening and diagnostic investigations. 4. Interprets reports, diagnostics and lab work and analyzes the information to monitor progress and plan treatment. 5. Consults with the physician(s) as per NP practice standards. 6. Works collaboratively and consults with the integrated health care professionals (IHP) at the Georgian Bay Family Health Team and community agencies. 7. Prescribes drugs within the statutory and regulatory standards, limits and conditions for Nurse Practitioners. 8. Provides leadership and support to the IHP’s and support staff at the family health team or clinic. 9. Participates in interdisciplinary staff and nursing education and professional development. 10. Other duties as assigned. SPECIFICATIONS 1) QUALIFICATIONS/RESPONSIBILITIES: a) Professional Baccalaureate Nursing Degree with Extended Class b) Certification from the College of Nurses of Ontario (CNO) c) Member of RNAO (preferred) d) Member of NPAO (an asset) e) Minimum of two years experience nursing experience in primary care as a Nurse Practitioner f) BCLS or ACLS g) Liability protection through Canadian Nurse Protective Society 2) COMPETENCIES/ SKILLS / KNOWLEDGE: a) Advanced knowledge of the nursing and consultative process. b) Knowledge of and practicing within the CNO practice standards for Nurse Practitioners. c) Demonstrated excellence in oral and written communication. d) Excellent interpersonal, problem solving and teaching skills. e) Ability to function collaboratively within an interdisciplinary team. f) Excellent critical thinking, decision making, problem recognition and solving skills. g) Demonstrates qualities of caring, compassion, integrity, responsibility and understanding, which reflect credit upon the profession. h) Knowledge and skill in utilizing electronic medical records. i) Adaptability and flexibility to changes in the work environment or locations. 3) EQUIPMENT USED: Various computer and office equipment, including Mac computers, printers, scanner, fax, photocopier, and telephone. 4) WORK ENVIRONMENT: GBFHT Office - The GBFHT is located at 186 Erie Street. The Nurse Practitioner will work both in the GBFHT office during administrative hours, as well as in the traditional Family Practice Setting, which are diverse across the South Georgian Bay Region. 5) REPORTING RELATIONSHIPS: The Nurse Practitioner reports to the Nurse Practitioner Team Lead and the Executive Director.
