Nurse Practitioner - Loyalist FHT (Amherstview)

Full time Nurse Practitioner Loyalist Family Health Team Overview: The Loyalist Family Health Team (LFHT) in beautiful Amherstview, Ontario is seeking a skilled, enthusiastic, dedicated full-time Nurse Practitioner interested in collaborative team based primary care. This position will be one half acute same day care and one half comprehensive adult care with a special focus on complex frail elderly and will include programming and home visits. Expertise and interest in dementia care and palliative care will be considered assets. This exciting opportunity will appeal to a self-directed professional interested in providing excellent team based care to a wide variety of patients in collaboration with a group of supportive physicians and skilled, committed interdisciplinary health care providers. Main Responsibilities:
  • Collaborates and/or consults with physicians and other interdisciplinary team members to provide excellent, patient-centred health care
  • Diagnoses and treats clients for acute episodic illnesses
  • Orders appropriate investigations; interprets reports of investigations and analyzes information to monitor progress and plan treatment
  • Applies knowledge of pharmacology and evidence based practice in appropriately selecting, prescribing, administering and monitoring the effects of drugs
  • Helps to develop and implement programs and measure program performance
  • In partnership with other providers; support and counsel patients, establish priorities for management of health conditions, provides follow-up treatment, communicates with patients and families about health findings
  • Liaises with community partners to optimize health care
Education, Experience and Skills:
  • Current extended class certificate and unrestricted registration with the College of Nurses of Ontario (Masters or NP-PHC are considered an asset)
  • Current membership with RNAO, including professional liability insurance program
  • At least two (2) years clinical experience is preferred, particularly in primary care
  • Strong interest in the care of older adults
  • Ability to work effectively independently and as part of a team
  • Certified Diabetes Educator will be considered an asset
  • Expertise in dementia care and/or palliative care will be considered an asset
Job Requirements:
  • Must be able to travel and hold a valid driver’s license
  • Willingness to work one evening per week until 8 pm is strongly preferred
