Nurse Practitioner - Humber River FHT (Toronto)

The Humber River Family Health Team (HRFHT), a growing, multi-site FHT, provides collaborative, interdisciplinary, comprehensive primary health care to a rostered population of patients with a range of complex healthcare needs and very diverse backgrounds in the northwest Greater Toronto Area (GTA).   The FHT is committed to holistic, compassionate, patient-centred care emphasizing disease and illness prevention, chronic disease management, harm reduction and health promotion. The HRFHT seeks a highly motivated Nurse Practitioner (NP) for a full-time position, reporting to the Executive Director. Purpose of Position: As a key member of an interdisciplinary team, the NP will provide comprehensive primary care to patients in the community, ensuring continuity of care.  The NP should be comfortable working to his or her full scope of practice, providing  care and services to patients independent of physician involvement, as well as assisting with physician-managed care.  In addition, the NP will assume leadership in the establishment and effective and efficient operation of a Well Woman and Child program clinic to assist women and their children who have difficulty accessing a primary care provider. The NP-Led program will deliver:
  • Holistic care that addresses all needs of each patient;
  • Enhanced health promotion;
  • Chronic Disease and illness management and prevention;
  • Care coordination and navigation of the health care system; and
  • Integrated care within the community through established partnerships.
The NP will contribute to educational, community relations, client advocacy and other organizational initiatives. Qualifications:
  • Registered Nurse with graduate level preparation in Primary Care, Pediatrics, or Adult Nursing from a recognized university program.
  • Certification Primary Health Care Nurse Practitioner Certificate & current BCLS Certificate
  • Member in good standing with the College of Nurses of Ontario as RN (Extended Class).
  • Membership in professional organization (s) in nursing such as RNAO.
  • Minimum 3-5 years clinical experience in a community health setting
  • Experience working in advanced clinical practice in primary care, preferably with women and children
  • Knowledge of best practices in programs and services for women and children with complex needs
  • Demonstrated ability to work effectively as a member of an Interprofessional team, with excellent interpersonal and communication skills
  • Working knowledge/skill with adult education techniques
  • Advanced knowledge/skill regarding patient safety, change leadership, project management and evaluation
  • Working knowledge of Electronic Medical Record (EMR) and required computer skills.
  • Able to work flexible hours (weekend and evening work required)
  • Valid Ontario’s driver’s license and access to a vehicle.
