Nurse Practitioner - Burlington FHT

Position:  Nurse Practitioner, Full Time Reporting to:  Executive Director &  Physicians (for clinical responsibilities) Close Date:  April 18, 2014 Contact info:  Joanne Pearson Position Overview Operating within their full scope of practice, the Nurse Practitioner will function in close collaboration with all of the Physicians and Allied Health Professionals within the recently established Burlington Family Health Team.  The NP will play a lead role in the continued development and ongoing management of a Seniors Aging at Home Program as well as other specialized programs related to the health of our Seniors population. Role Specific Expertise The NP will demonstrate a high level of autonomy and expert skill in formulating clinical decisions and appropriately managing a population of elderly patients
  • Assumes a leadership role in the Aging at Home Program in conjunction with our Occupational Therapist
  • Collaborates with Physicians and AHP’s in the development of other FHT programs
  • Participates fully in the delivery of comprehensive primary care
  • Assists with Quality Improvement Initiatives
  • Provides educational support to patients, families and/or caregivers as well as medical residents
Key Qualifications
  • Minimum of 1 year, recent, relevant primary care nursing experience preferably in geriatric care
  • Current registration with the College of Nurses as an NP- Primary Health Care
  • Excellent health assessment/clinical skills
  • Experience in program development and management
  • Knowledge of and experience with computer systems and applications
