 NURSE PRACTIONER – FULL TIME

Bridgepoint Family Health Team provides primary care to patients from the Riverdale community and across the GTA. Our team uses an innovative service model that includes the patient in their care. We have a dynamic interdisciplinary team working closely together to provide the best care possible. Our interdisciplinary team includes Physicians, Nurse Practitioner, Registered Nurses, Social Worker, Dietitian and Medical Assistants. We are currently seeking a full time Nurse Practitioner. The Nurse Practitioner is a regulated member of the health care team who utilizes the nursing process, critical thinking, problem-solving and decision making skills to enhance care to individuals, families, groups and communities across the lifespan. This position collaborates with patients and other health care professionals to enhance the care for patients with mental health diagnoses. The Nurse Practitioner provides professional guidance to other health professionals and practices autonomously and interdependently within the context of an interdisciplinary health care team, making referrals to specialist physicians and others as appropriate. The ideal candidate will have:
  • Current registration from the College of Nurses of Ontario as an RN (EC);
  • Completion of Master’s Degree in Nursing would be an asset;
  • RNAO mental health certificate would be an asset;
  • Minimum 3 years recent experience in a primary care setting;
  • 1 to 2 years experience providing mental health support and services;
  • Previous experience in Case Management would be an asset;
  • Experience in the development and maintenance of Medical Directives;
  • Effective patient/family counselling skills;
  • Demonstrated ability for independent action, initiative, problem solving, organization and prioritization;
  • Excellent communication, negotiation, teaching and interpersonal skills with the ability to work well with patients, families, staff and community;
  • Proficiency in the use of personal computer and software programs including Microsoft Office, email and internet. Experience in Electronic Medical Records would be an asset;
  • Demonstrated ability to be flexible and adapt positively to constantly changing clinical situations;
  • Demonstrated commitment to continuing professional development;
  • A strong desire and capacity to promote professional practice and excellence in evidence-based, patient centered care.
