Job Description
As a member of the inter-professional primary care, Community Care Team (CCT), the Health Promoter is responsible for the planning, implementation, operation and evaluation of health promotion, health education and community development/capacity building programs, services, events and activities at Health for All Family Health Team. The Health Promoter will build and support linkages in the community and foster and maintain meaningful partnerships with other organizations, community groups and stakeholders as part of expanded Interprofessional Primary Care Team (IPCT) programs and services.

The Health Promoter will collaborate with the Executive Director, CCT Clinical Lead, Clinical and Administrative Teams and staff on the provision of relevant health promotion initiatives, programs and services to clients and community members. 

Reporting:
The Health Promoter is a member of the CCT Clinical Team and reports to the Executive Director.

POSITION REQUIREMENTS 

Qualifications

  • Graduate degree from a recognized university in relevant discipline, or combination of undergraduate degree and 3-5 years related experience
  • Experience in design, development and delivery of health promotion projects, using a community development approach
  • Experience in facilitating groups effectively both in community and within health service organizations
  • Experience working with individuals from differing social and cultural backgrounds in clinic and community settings
  • Experience and skill in group program planning, implementation and evaluation frameworks
  • Demonstrated abilities in outreach and community engagement
  • Experience in program development, implementation, monitoring, and evaluation
  • Experience working with marginalized populations
  • Experience in project management is an asset
  • Experience in chairing community advisory and patient committees

  • Ability to identify and manage program resources
  • Broad knowledge in the areas of public health, population health, adult education and health promotion
  • Proficient in utilizing a wide range of social media platforms with a keen understanding pf their unique features and audience engagement techniques
  • Demonstrated expertise in creating captivating visual content such as flyers, engaging social media posts, to effectively enhance public awareness and promote team programs
  • Excellent problem-solving, conflict management and time management skills
  • Fluency in a second language from the priority communities of Markham and Eastern York Region is an asset
  • Knowledge of issues and challenges facing low income, vulnerable and diverse communities
  • Strong knowledge equity, diversity and inclusion principles and strategies
  • Requires flexible hours, including evenings and weekends
  • Skilled with computers including knowledge of social media platforms, presentation software
  • Access to a reliable vehicle and a valid driver’s license is an asset
