Executive Director- Humber River FHT (Toronto)

Family Health Teams use a patient-centred approach to primary health care that brings together physicians, a team of inter-disciplinary health care providers and administrators to coordinate the highest possible quality of care for patients. Family Health Teams are a major component of Ontario’s health care plan that will improve access to primary health care across the province. The idea for the Humber River FHT began in January 2011 and a decision was made to develop four sites under the umbrella of the Humber River FHT: Black Creek (Black Creek and Eglinton), Dixon (Dixon and Islington), Finch (Jane and Finch), and Wilson (Keele and Wilson). The Humber River FHT, located within the Central LHIN, will be providing services to patients located in the northwest GTA. This is a very diverse community; 40% have emigrated here from over 140 countries and together speak more than 70 different languages. Executive Director

We are looking for a dynamic leader with demonstrated management / administrative skills to support the growth and development of this Family Health Team. The Executive Director position is central to the successful operation of the Humber River Family Health Team.  It is a position of strong leadership, requiring excellent interpersonal and communication skills, strategic planning and team building capability along with financial and administrative expertise.  The Executive Director reports to the Board of Directors and will work with the Board to develop strategic direction for the Family Health Team. They will provide formal progress reports on the implementation of these plans to both the Board and Ministry of Health. The Executive Director also ensures that the appropriate policies (set by the Board), procedures, systems, resources and other infrastructure are in place to sustain a patient focused, collaborative and a results-based organization.  The Executive Director also develops strategic partnerships with the Humber River Hospital and community based organizations to help deliver its vision, mission and strategic objectives. In fulfilling Humber River Family Health Team’s mandate, the Executive Director works in close collaboration with the Lead Physician to ensure that primary health care objectives are achieved. A successful track record of organizational management at the senior level in the health care sector with financial experience backed by a graduate degree in a business or health-related discipline, or a suitable combination of education and experience will be an asset.
