Family Health Teams use a patient-centred approach to primary health care that brings together physicians, a team of inter-disciplinary health care providers and administrators to coordinate the highest possible quality of care for patients. Family Health Teams are a major component of Ontario’s health care plan that will improve access to primary health care across the province. The idea for the Humber River FHT began in January 2011 and a decision was made to develop four sites under the umbrella of the Humber River FHT: Black Creek (Black Creek and Eglinton), Dixon (Dixon and Islington), Finch (Jane and Finch), and Wilson (Keele and Wilson). The Humber River FHT, located within the Central LHIN, will be providing services to patients located in the northwest GTA. This is a very diverse community; 40% have emigrated here from over 140 countries and together speak more than 70 different languages. We have a vision of improving the lives of our patients for a healthier community; ‘Making a Difference’. We are building our ability to deliver a model of lifelong care that optimizes health for the individuals and families in the communities that we serve.
