Executive Director – Credit Valley FHT (Toronto)

The Credit Valley Family Health Team (CVFHT) is a vibrant bilingual academic FHT affiliated with Trillium Health Partners and the University of Toronto.  There are 2 sites. They share collectively 10 Family Physicians, 24 residents, nurses, a nurse practitioner, social workers, a pharmacist, dietitians, a chiropodist, two diabetes teams and a stable and capable administrative, support and clerical staff. The CVFHT’s Mission of Excellence is to provide high standard interprofessional primary care and family medicine teaching. We are currently seeking a full time Executive Director to co-lead our team in collaboration with our Executive Committee (Executive Director, Lead Physician, Site Director of the Teaching Unit), in achieving our Mission and Strategic Plan. General Responsibilities:
  1. With the Executive Committee of the Board, the Executive Director is responsible to lead the FHT team professionals, support and administrative staff. The Executive Director has day to day operational management of the Team and its human resources.
  2. The Executive Director represents and promotes the activities and requirements of the Team to partner with health care organizations and leads liaison with the Local Health Integrated Network and the Ministry of Health and Long Term Care.
  3. Overseeing and actively participating in Quality Improvement and Risk Management activities;
  4. Representing the organization at the LHIN, community, governmental, and provincial levels.
Specific Responsibilities:
  1. Direct responsibility for, or valued collaborator in, all aspects of the organization
  2. With support from current team resources and leadership:
    1. Supervises and reports to the Board of Directors necessary plans and tasks for sound financial management
    2. Supports, advises and plans so that the Quality Improvement Plan is current and capable of achieving its goals
    3. Supervises Policy and Procedures to accomplish clear internal communication
    4. Assesses Team growth and recommend strategies for continued growth as such is prudent and consistent with the Strategic Plan
    5. Underpins the administrative performance necessary for a successful resident teaching program
Required Qualifications and Experience
  1. A member of a Regulated Health Care Profession in the Province of Ontario
  2. Post graduate education, as a Masters or similar academic experience related to health care management
  3. 5 years with experience at a manger or more senior level; primary care field an advantage
  4. Clinical credibility and experience to lead a high functioning health care team
  5. Interpersonal skills to manage a varied staff and maintain a harmonious and high functioning team internal environment
  6. Creativity and flexibility to react to an ever changing external environment and with other leadership, adapt and exploit such changes for the Team benefit
  7. Computer literacy to work in the context of the FHT’s Electronic Medical Record, ministry of health and financial information systems.
