The Credit Valley Family Health Team Board of Directors is seeking a demonstrated dynamic and innovative health care leader to serve in the position of Executive Director. The Credit Valley Family Health Team is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging multi-disciplinary primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities. The Executive Director role also has an exciting growth opportunity, as the team has recently received $1.3M additional base funds to expand the Interprofessional Care Team (IPCT) model into the North West Mississauga Care Community. The successful candidate will be an experienced and seasoned health care leader with demonstrated excellence in executing in a lean organization. They will be working closely with the Board of Directors in setting the future direction of the organization through the upcoming cycle of strategic planning. The successful candidate will have a clear vision on how to articulate the strategic plan into deliverable key activities. They will be experienced in program development, measurement, and quality improvement methodologies. They will demonstrate in their previous work impeccable integrity and excellent communication skills with employees, stakeholders and community members. Key Duties and Responsibilities


  • To provide effective strategic leadership, guidance and management to the CV FHT staff including human resources and financial resources management
  • To establish a culture of excellence in patient care and customer service and collaborate with the CV FHO to deliver this across the organization
  • To work closely and collaboratively with the CV FHO to effectively and efficiently manage the day to day operational activities of the CV FHT as a means

Financial Management

  • To establish and maintain a financial management framework that supports the decision making capabilities of the CV FHT and makes use of the appropriate financial systems to ensure the CV FHT continuously operates within its approved business plan and allocated funds

Organizational Management

  • To empower and inspire the team to conceive, develop and plan programs the CV FHT can deliver to the benefit of patients and the broader community
  • To provide balance organizational priorities by facilitating the efficient function of the organization and the attainment of strategic goals
  • To position the CV FHT to be responsive to the changing health care environment and the needs of the population


  • To act as the Privacy Officer and ensure adherence to the freedom of information and protection of privacy


  • To engage in two way communication with all team members to create a cohesive team and seamless collaboration with the CV FHO
  • To communicate effectively with all patients, families, peers, other health care professionals and community partners
  • To act as the spokesperson for the FHT as delegated by the Board of Directors

External Partnerships

  • To actively participate with health care planning processes occurring at the neighbourhood, care community (Sub-Region) and LHIN level
  • To establish and foster collaborative working relationships with local organizations in the Community Care Sector (CCS) and Social Services Agencies (SSA)


  • To act as the Secretary (ex-officio) to the Board of Directors and also to report on monthly basis to the Board on operational risk issues and strategic direction of the FHT
  • To lead the organization and the Board in a strategic planning process which will set the strategic direction for the next 3-5 years
