Dietitian - Burlington FHT

Part Time Contract position:  June 2014-May 2015 .5 FTE The Burlington Family Health Team (FHT) is seeking a Dietitian who is a dynamic team player to join an innovative, community-based primary health care team. The Burlington Family Health Team  is a Wave 5 multi-site, fast-growing FHT that opened in July 2011. We provide comprehensive, collaborative primary care to Burlington and catchment area, serving 9,500 patients through a strong interdisciplinary team comprised of Social Workers, a Nurse Practitioner, Nurses, a Pharmacist, Occupational Therapist and a Dietitian. The Burlington FHT has a clinical teaching affiliation with McMaster Family Medicine. Working as a Dietitian on a multidisciplinary team, you will:
  • Work collaboratively with other IHP’s to provide comprehensive care plans for our patients
  • Be involved in health promotion and education for our patients and team members
  • Provide nutrition counselling and develop specialized nutrition therapy to address nutritional related illnesses
  • Conduct nutrition assessments, develop nutrition care plans, and monitor effectiveness of the nutrition care plans through follow-up visits
  • Facilitate group education classes
  • Be a key member of the diabetes and chronic disease management programs and support other Quality Improvement initiatives and programs
  • Be a resource to family doctors, other allied health professionals, medical residents and other academic facilities
Your profile will include:
  • Baccalaureate of Science Degree (or equivalent) in Foods and Nutrition from a recognized university and completion of Internship Program accredited by Dietitians of Canada
  • Member in good standing with the College of Dietitians of Ontario and proof of liability insurance
  • Member of Dietitians of Canada preferred
  • Ability to work independently and collaboratively within an interdisciplinary health care team
  • Experience in group facilitation and individual counselling
  • Strong interpersonal and organizational skills
  • Proficiency with an EMR system for charting (OSCAR system experience, an asset)
  • One year of experience in community or primary care setting an asset
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment
  • Excellent oral and written communication skills
