Diabetes Educator – Beamsville Medical Centre FHT (Beamsville)

Diabetes Educator Beamsville, ON, CANADA Beamsville Medical Centre Family Health Team (FHT) is a rural health centre in the Niagara Region dedicated to offering exemplary comprehensive health care. We provide a patient-centered approach with a focus on chronic disease management, disease prevention and health promotion. We are currently in the process of seeking a Registered Dietician or Registered Nurse to fill the position of Diabetes Educator. The position offered is a temporary part time position (0.3 FT hours/ 12 hours per week) for maternity leave coverage. Starting the week of January 6, 2013. Contract will last the duration of 12 months with the possibility of extended contract. Position: As a key member of the Beamsville Medical Centre FHT you will coordinate the Diabetes Education Program (DEP) by directly caring for and educating diabetic patients and their families.  You will be an advocate for the dignity and self-respect for people living with diabetes. In collaboration with the physicians and other allied health care professionals you will design, develop, implement and evaluate education, care and treatment approaches for individuals and groups in accordance with the 2013 CDA Clinical Practice Guidelines. You will also provide direct patient care, which includes independently making day-to-day decisions regarding individual patient care and treatment as delegated by medical orders. Clientele include but are not limited to adults living with diabetes, family members and other healthcare providers.  Accepts referrals from 9 primary care physicians. DEP activities and patient care will be in accordance with FHT policies/procedures, professional practice standards and required legislation (e.g. Nursing Act, Regulated Health Professions Act). Roles and Responsibilities Patient Care
  • Assists patients living with diabetes to achieve their optimum level of health in situations of normal health, illness, injury or through the process of dying
  • Provides direct patient care including assessments, planning, intervention and monitoring (focus is on prevention of complications and management of stable diabetes)
  • Collaborates closely with physicians and FHT health professionals on patient care
  • Partners with patients and their families to enable them to design and manage their own individualized diabetes protocols/plans (e.g. establishes goals with patients)
  • Evaluates patient comprehension and compliance to a care plan by reviewing clinical and learning outcomes
  • Assesses patient educational needs by identifying physical, social and or emotional needs that may influence the education process and the long term outcomes
  • Schedules follow up appointments
  • Plans, develops and delivers education related to diabetes (e.g. diabetes prevention, medication use, diet and nutritional monitoring, prevention and care of complications, foot care, blood glucose monitoring and the techniques of self-monitoring and injection)
  • Collaborates with other FHT health professionals to provide exceptional education
  • Responsible for overall delivery of DEP patient services
  • Provide DEP leadership in consultation with FHT physicians and FHT Administrator
  • Monitor supply levels and/or expiration of DEP medical supplies
  • Participate in clinical clerks/family practice residents supervision by providing DEP educational opportunities
  • Review new treatment modalities introduced by pharmaceutical companies
  • Actively participate in interdisciplinary, staff, team and committee meetings
  • Develop and maintain liaisons with other community agencies

Maintain own academic qualifications and participate in self-directed learning to ensure his/her practice remains relevant (by attending professional conferences, e-learning and journal reviews).  Stay current and aware of opportunities to implement new, evidenced-based methods of patient assessment and treatment. Qualifications
  • Current registration with the College of Nurses of Ontario or College of Dieticians of Ontario
  • Certified Diabetes Educator  (CDE) with the Canadian Diabetes Educator Certification Board
  • Current Basic Cardiac Life Support (BCLS) certification required
  • Minimum 2 years of clinical experience as RN or RD, preferably in community-based health care
  • Minimum 1 year health teaching experience, and experience applying advanced diabetes knowledge, physical assessment skills, diagnostic intervention and self-care management
