Data Analyst / Interpretation – Barrie Community HealthLink 12 Month Contract – Full Time

About Barrie Community HealthLink Ontario is improving care for seniors and others with complex conditions through HealthLinks. This innovative approach brings together health care providers and key partners in a community to better, and more quickly, coordinate care for high needs patients. HealthLink is a new model of care where all providers in a community, including family care providers, specialists, hospitals, long-term care, home care and other community supports, are charged with coordinating plans at the patient level. This will help improve patient transitions within the system and help ensure patients receive more responsive care that addresses their specific needs with the support of a tightly knit team of providers. As the HealthLink grows and develops, there is an opportunity for the Barrie HealthLink to be a leader in information interpretation and performance reporting, and initiate a HealthLink data interpretation role. Position Description This role will be dedicated to information management and data analysis of the Barrie HealthLink. The primary purpose of the position will be customer service and information needs to HealthLinks and High User strategies.  This position will also act as a resource for other HealthLinks and will share best practices where appropriate. The individual in this role will report to the Director of HealthLink Barrie and accountable to the Steering Committee and the Data and IM Working Group. Primary Responsibilities & Deliverables
  • Work towards Advanced Technology: By building the beta versions of the scorecards and metrics, the ground work performed in this role will provide the necessary foundation for advanced methodologies around data collection and technology. A key to any successful strategy implementation is to have a solid framework of indicators, targets, benchmarks and performance corridors. This position will understand the in-depth business operations of the HealthLink, leveraging this to guide development of tools that will meet the information demands both from an overarching performance framework and a day to day operational need.
  • Administer Patient Satisfaction: This position can develop and customize patient satisfaction surveys, as well as collect and maintain data that can be reported to the senior leadership team and appropriate steering committees.
  • Patient Experience: This position will work with the LHIN and HQO to develop Experienced Based Design tools and a framework for the Barrie area.
  • Ensure Synergy between High User Groups: There are currently several “High User” strategies in both Ontario and NSM LHIN. This role can act as a common denominator between the various groups, insuring synergy at the data level between projects where current connections may be weak. This role can be the strong tie in data interpretation between Simcoe County EMS, NSM CCAC, NSM LHIN, BCFHT, RVH, and potentially evolve to other primary care groups, other providers and HealthLinks This position will also serve as a model role for other HealthLinks, demonstrating the “art of possible” around data interpretation, information sharing and effective performance management.
  • Develop Clinical Priorities: With the support of other partners and with information sources supported and leveraged by RVH DSU, the Analyst position would be able to maintain and update tools that will help the Barrie HealthLink refresh current clinical priorities, as well as ensure alignment with other strategies within the LHIN such as the Regional Home and Community Care Decision Support Task Group, as well as continue to report on outcomes, and demonstrate results and efficacy of the HealthLink.
  • Reporting on Indicators: Fulfill Legislative Reporting Requirements (i.e. identified in funding letters, accountability agreements and legislation, as well as work with steering and executive teams, as well as other subject matter experts on the development of additional indicators to track HealthLink performance.
  • Produce Regular Performance Reporting: Provide the Steering Committee and Working Group regular performance updates.
  • Maintain Roster / CRM for BCHL: Develop and maintain a roster and customer / client (patient) relationship management (CRM) tool. The thought behind this tool is it can serve as a platform for current patients and provide information to all stakeholders in the care of these clients, as well as provide additional data points that can be used in the future for refining clinical priories and client profiles.
  • Improving Data Quality: This position will work with all partners to work towards improving the data quality in the current system and on a go forward basis.
Education, Qualifications and Skills
  • Degree in Math or Sciences or equivalent experience demonstrating broad I.M knowledge
  • Minimum 2 years of experience in data analysis, reporting and performance measurement and tracking
  • Working knowledge of the Personal Health Information Protection Act 2004 preferred
  • Experience in health care involving data analysis and reporting preferred
  • Proven ability to be deliver to tight deadlines, alone and in teams
  • Strong demonstrated interpersonal and influencing skills
  • Strong knowledge of problem-solving techniques and facilitation skills
  • Demonstrated proficiency in MS Office, especially Excel
  • Experience in other statistical reporting tools (Preferred)
  • Ability to demonstrate flexibility and adapt to changing priorities and expectations
  • Strong presentation skills related to the sharing of information (verbal, written, etc)
