Consulting Pharmacist -The West Durham Family Health Team

Job Description:

We are searching for an energetic individual who is enthusiastic about working in a community-based primary care setting. The Pharmacist will be a key member of an interdisciplinary team of health care providers including Physicians, Nurses, Nurse Practitioners, Dietitian, Occupational Therapist and Social Worker.

Job Summary:
Working in collaboration with an interdisciplinary team, the Pharmacist practices patient-centred care to achieve optimal drug therapy outcomes. The Pharmacist uses unique knowledge and skills to prevent or resolve actual and potential drug-related problems. The Pharmacist also participates in the development, implementation, and facilitation of various patient educational programs. 

Qualifications and Experience:

• Bachelor of Science in Pharmacy from an accredited University
• Registration with the College of Pharmacy in Ontario
• PharmD, or hospital residency or equivalent experience
• Minimum 5 years related or applicable experience as a pharmacist in a community, hospital, or primary care setting
• Additional training or credentialing in pharmacotherapy – Certified Diabetes Educator (CDE) or Certified Geriatric Pharmacist is preferred
• Experience in program development, implementation, monitoring and evaluation

Roles and Responsibilities:

The role and responsibilities of the Pharmacist include but are not limited to:

• Individual Patient Assessment: To identify, prevent and resolved medication-related issues 
• Consultation based on referrals from Interdisciplinary team members – including Medication Reconciliation, provide drug-related recommendations, or evidence-based information or education to team members
• Design and facilitate education programs to patient, caregivers, team members on various therapeutic topics
• “Seamless” Care – Communicate with hospital and community pharmacists and other team members as required to ensure smooth transition for medication related care between care sites.
• Recommend improvements to Medication Use Process including drug prescribing, handling of drug samples, documentation, etc.
