Full Time (Maternity Leave)
Family Health Teams (FHTs) use a patient-centered approach to primary health care that brings together doctors, nurse practitioners, registered nurses and other allied health professionals to coordinate the highest possible quality of care for patients – when they need it and as close to home as possible. FHTs are a key component of Ontario’s health- care plan to reduce wait times, improve access to doctors and nurses, and keep Ontarians healthy.
The Burlington Family Health Team (six physicians) is seeking a dynamic team player to join an innovative, community-based primary health care team.
We are looking for a Social Worker to fill a nine (9) month maternity leave that will commence in March 2012.
As a member of a multidisciplinary academic team, you will provide therapy to a wide range of patients with mild to severe mental health issues through individual and couple/family counselling and be a resource to family doctors, other allied health professionals, medical residents and other academic facilities.
Requirements
• Masters degree in Social Work
• Registered with Ontario College of Social Workers and Social Service Workers and/or Ontario Association of Professional Social Workers.
• Ability to work independently and as part of a collaborative team
• Expertise in working with mental health diagnoses such as mood, and personality disorders, concurrent disorders and anxiety disorders.
• Experience with delivering specific therapeutic approaches such as cognitive behaviour therapy, substance abuse treatment, brief solution focus approach etc.
• Experience with dialectical behaviour therapy an asset.
• Experience in assessment and counselling in a primary care setting
• Experience in program development, facilitation and evaluation
• Strong interpersonal and organizational skills
• Interest or experience in teaching and mentoring students
• Proficient with an EMR system for charting. (OSCAR system experience an asset)
• Ability to prioritize, manage time effectively and be flexible in a very active work environment
• Excellent oral and written skills
