Dufferin Area Family Health Team- Full-time Nurse Educator Position

REGISTERED NURSE EDUCATOR

The Registered Nurse Educator is a vital part of DAFHT and will collaborate with the physicians and other interdisciplinary providers to achieve the vision and mission of the organization. The RN will share expertise and knowledge of chronic disease management and optimizing health with clients and their families, as well as other health care professionals within the FHT.

DUTIES AND RESPONSIBILITIES

  • Screens, prioritizes and processes client referrals.
  • Coordinates the assessment of client health status and its impact on management of chronic disease with other members of the interdisciplinary team.
  • Creates a built-in recall system that will track patients with one or more risk factors and notify for re-screening and assessment.
  • Works in collaboration with interdisciplinary team to safely and effectively provide appropriate primary care to clients.
  • Reviews client clinical information obtained through physician referral, laboratory reports and health records.
  • Conducts consultations with clients to obtain relevant information and assess client health learning needs.
  • Collaborates with Family Physician, Nurse Practitioner and other team members on developing appropriate individualized care plans with the patients based on best practices.
  • Reviews current literature/resources to support preparation of education materials and tools including liaising with community partners.
  • Documents using client health record, refers to other resources and arranges follow-up as necessary.
  • Manages, reviews and makes recommendations regarding protocols for referral of FHT patients to a chronic disease management program.
  • Practices within the scope and according to standards of practice as outlined by the College of Nurses of Ontario.

EDUCATION/EXPERIENCE:

  • Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred.
  • Current registration with the College of Nurses of Ontario.
  • Current membership with Registered Nurses Association of Ontario preferred.
  • Basic CPR certification.
  • Current Ontario Drivers’ License.
  • Demonstrated experience in Community and/or Primary Care Setting
  • Demonstrated experience in the area of chronic disease management and adult education.

SKILLS/ABILITIES

  • Excellent oral/written skills
  • Ability to work effectively with all members of the health care team.
  • Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery
  • Knowledge and proficiency to develop and conduct educational programs for learners, and experience in evaluation of learning activities specific to individual performance.
  • Ability to develop strategies that successfully meet the needs of diverse patient, family and provider population.
  • Proficiency in the use of computer hardware and software.
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment.
  • Willingness to teach and train other members of the interdisciplinary team or students as required.
