Purpose of the Position:
The Inner City Family Health Team is a multidisciplinary team which serves homeless clients with complex health needs. These include untreated chronic and complex multi-system diseases, an elevated risk of acute health emergencies, and serious and persistent mental health and substance use challenges. Clients are predominantly current or former users of Seaton House Shelter. Staff will work at our clinic on Queen Street, as well as at nearby Seaton House and other community locations on an episodic basis. Services are provided by the team using a holistic, client-centred model of care with an emphasis on disease prevention, chronic disease management, harm reduction and health promotion.
The Registered Nurse collaborates with Inner City Family Physicians and Psychiatrists, our interdisciplinary team, and community and shelter staff to provide comprehensive health services, including assessment, primary nursing care, case coordination, health education and health care management to clients experiencing chronic diseases, complex multi-system diseases, and mental health and/or substance use challenges.
Roles and Responsibilities:
- Provides nursing care and treatment through pharmacological, complimentary therapies and/or counseling interventions to a marginalized population presenting with complex health needs. These may include (often previously untreated) wounds, skin disorders, infectious diseases and mental health issues.
- Dispenses and administers medication/drugs as prescribed by the NP, Family Physician and/or Psychiatrist to treat diseases or disorders within the RN’s scope of practice.
- Provides individual client assessments (physical, emotional and psychological), including client history, recording of vitals, biometric measurements, and triage of clients based on level of need.
- Collaborates with clients, physicians, psychiatrists and other allied health members of the team to develop appropriate, client-centred, individualized care plans.
- Engages clients in risk factor and health problem identification, goal setting and making choices of intervention for disease treatment, prevention and health promotion.
- Sees clients at the clinic with scheduled and urgent appointments, and at Seaton House and in the community during regular and extended hours (as determined by the FHT).
- Undertakes home visits and outreach services as required and appropriate.
- Keeps complete and accurate records of client contacts and care through timely documentation in the Inner City Family Health Team Electronic Medical record (EMR).
- Contributes to the development of and implements strategies that promote health, prevent disease, and foster independence and healthy living for Inner City FHT clients.
- Contributes to the development of learning resources and participates in health education.
- Actively participates in staff, team and other Family Health Team meetings as appropriate/required.
- Participates in clinical projects/studies/evaluations as required.
- Supports the ICFHT with related health care organizations to promote coordination and/or planning of local health care services.
- Some evening and weekend work may be required.
QUALIFICATIONS, SKILLS and EXPERIENCE:
- A Bachelor of Science in Nursing
- Current Registration and member in good standing with the Ontario College of Nurses as a Registered Nurse.
- Current Membership with the RNAO and evidence of liability insurance.
- Knowledge and a demonstrated ability to provide nursing care in community health and public health sciences.
- 3 years of clinical experience in working with clients in community based health/primary care settings.
- Current experience in chronic disease management and with client population with serious and persistent mental illness and/or addiction.
- Experience working with homeless service users preferred.
- Knowledge of and commitment to client–centred philosophy and basic principles and practice of community service delivery and chronic disease management models.
- Knowledge of community resources relevant to the provision of health promotion and illness prevention.
- Must have current CPR certificate.
- Open minded, non-judgmental with a commitment to upholding the rights and dignity of all people and respecting their diversity.
- Knowledge and proficiency in current, evidenced-based methods and practices in health care delivery, with an emphasis on mental health, health promotion and risk reduction.
- Excellent verbal and written communication skills.
- Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook.
- Knowledge and experienced in the application of electronic medical records (EMR).
- A sound understanding of and commitment to the Social Determinants of Health, and the ability to effectively participate in the efforts of an interdisciplinary team to contribute to comprehensive health care.
