Nurse Practitioner (EC) Primary Health Care Part-time

The Nurse Practitioner is an integral part of an interdisciplinary team of health professionals. As a vital member of the team, the Nurse Practitioner (EC) will deliver quality care to patients with a focus on increasing patient access to health assessment, education, disease prevention, and support for chronic health conditions. The Nurse Practitioner will practice within his/her full scope of practice as outlines in the Standards of Practice by the College of Nurses of Ontario.

Education:

  • Current registration with the College of Nurses of Ontario as a RN-EC (extended class) – Primary Health Care
  • Current membership in professional associations:
    • Registered Nurses Association of Ontario (RNAO)
    • Nurse Practitioners’ Association of Ontario (NPAO)
  • Professional liability insurance coverage as referenced
  • Current CPR or Basic Cardiac Life Support (BCLS) registration

Experience and Qualifications:

  • Minimum 3-5 years of experience providing primary care to patients of all ages in a comprehensive Family Practice or relevant Ambulatory care setting
  • 3-5 years of experience providing patient care across the full spectrum of problems encountered in Family Medicine/Primary Care.
  • Ability to work in an inter-professional team environment and implement best practices in patient care
  • Previous experience with an Electronic Medical Record or willingness to learn electronic record keeping
  • Highly developed ability to organize time and set priorities effectively
  • Effective verbal and written communication skills

Roles and Responsibilities
The Nurse Practitioner’s Roles and Responsibilities may include the following:

Patient Care

  • Perform patient health assessments and health maintenance activities (e.g. history and physical, wellness exams, pap tests and breast exams, prenatal care, well baby visits and immunizations)
  • Monitor the ongoing therapy of patients with chronic illnesses by providing pharmacological and counseling interventions
  • Screen for the presence of chronic illnesses
  • Conduct examinations using an age/sex specific list of recommended preventative interventions and risk assessment
  • Diagnose and treat acute/episodic illnesses and injuries
  • Offer phone consultation/triage for acute/episodic illness
  • May conduct home visits for the frail elderly/geriatric patients with multiple medications and administer immunizations
  • Consistently record and document in the patient’s chart and/or electronic medical record (EMR)
  • Provide communication to hospital and community staff and other inter-professional team members as required to ensure smooth transition for care between care sites

Education and Advocacy

  • Carry out health promotion group and individual counseling/education
  • Participate in the development and evaluation of policies, protocols and procedures
  • Prepare MOH/LTC Service Reports
  • Other Activities – other activities may be assigned in collaboration with the inter-professional team

  • Must possess a dependable, consistent and reliable work record
  • Capacity to adapt quickly to a fast paced, dynamic work environment
  • High degree of accuracy and attention to detail
