Free Registration for Family Health Teams:
The Ontario Lung Association, the Primary Care Asthma Program, the Association of Family Health Teams of Ontario, Health Quality Ontario and Association of Ontario Health Centres will be hosting a Respiratory Health Forum.
The January 25th sessions are for people who are just starting respiratory health programs or planning to initiate them. Click here for the preliminary agenda.
The January 26 sessions are for those who wish to enhance already existing respiratory programs. Click here for the preliminary agenda.
The location will be the Toronto Eaton Centre Marriott Hotel. The Respiratory Health Forum is being provided through funding by the Ministry of Health and Long-Term Care, Asthma Plan of Action.
All Family Health Teams and Community Health Centres in the province will be invited to send one representative to this event – ideally, someone in your team who has a lead role in developing or leading a respiratory care program for your patients. Depending on the number of Teams/CHCs which sign up, we may be able to accommodate more than one person per team. We will be happy to put additional names on a waiting list.

Our goal is to provide an opportunity for you to learn from other FHTs and CHCs and other multidisciplinary settings (e.g. Primary Care Asthma Program sites) about how to establish a respiratory care program, or enhance the one you already have.
