Respiratory Health Forum – Save the Date!
TO ALL FAMILY HEALTH TEAMS:
Ontario Lung Association, Quality Improvement and Innovation Partnership, Primary Care Asthma Program and the Association of Family Health Teams of Ontario will be hosting a FREE one-day forum on respiratory health on January 26, 2011 at the Toronto Eaton Centre Marriott Hotel, through funding provided by the Ministry of Health and Long-Term Care.
All Family Health Teams in the province will be invited to send one representative to this event – ideally, someone in your team who has a lead role in developing or leading a respiratory care program for your patients.
Our goal is to provide an opportunity for you to learn from other Family Health Teams, and other multidisciplinary settings (e.g. Primary Care Asthma Program sites) about how to establish a respiratory care program, or enhance the one you already have.
Your input is extremely important to us to ensure that this learning event will meet your needs. We are therefore asking you (or your designate) to complete the attached needs assessment survey. It should only take about 10 to 15 minutes of your time.
As an incentive to participating in the needs assessment, we are offering a free registration to the OLA’s annual Better Breathing Conference in Toronto, January 27-29, 2011 to be drawn randomly from those who complete the survey.
