The Cardiac Care Network of Ontario (CCN) is pleased to be hosting the Provincial Heart Failure Strategy Forum on Thursday, February 13, 2014 at Toronto Marriott Bloor Yorkville Hotel—Forest Hill Room from 7:30am to 3:30pm. Space is limited so register soon to avoid disappointment.


The current system of heart failure (HF) care is fragmented, with gaps in transitions between acute and primary care, variations in access to care providers, and a lack of standardized tools and resources, all of which contribute to sub-optimal care. To address these challenges, CCN has developed a provincial strategy to enhance services in Ontario and to support optimal HF care. The CCN Provincial Heart Failure Strategy Forum will be an opportunity for clinicians and administrators across the province to come together to discuss the unique challenges as well as potential solutions to the delivery of optimal heart failure care in Ontario.


By the end of the forum, participants will:

  1. Develop a clear understanding of CCN’s Provincial Heart Failure Strategy;
  2. Understand the current state of heart failure care coordination and delivery in Ontario;
  3. Build a network of heart failure providers in their local community and across the province;
  4. Have an opportunity to take an active role in discussing solutions to address system challenges.

Who should attend?

Any clinician or administrator involved in the management of heart failure patients in Ontario.


Admission to the conference is complimentary, breakfast and lunch will be provided. Attendees are responsible for their own travel and accommodation.
