There is a critical interface at transitions between hospital and primary care making it crucial for best quality and continuity of care. While there are varied examples of successful models and approaches, there are still significant challenges that all partners face. Increasing collaboration is required to define key points of interaction and improved solutions for integration.

With imminent changes in policy proposed by the Price-Baker Report and Patients First, and in the broader context of health system reconfiguration, now is the time to dialogue and share as Ontario moves forward.
