System Navigator (Term) – Couchiching FHT (Orillia)

Position:  System Navigator (Contract for Maternity/Paternity Leave, Full-Time)
Reports To:  Executive Director

Position Summary:

The System Navigator will work with patients, their caregivers and the health care team to assure an integrated journey through the health care and community sectors.  As an expert in community resources, you will guide patients through the system and will advocate on their behalf.

Roles and Responsibilities:

  • Assess client’s needs, identify mutual goals and develop a service plan to navigate through the health care environment
  • Provide referrals for patients to agencies that may provide financial assistance, medical treatment, legal aid, housing, transportation, day care, meals  and other services
  • Liaise with community agencies or partners and identify additional or alternative services
  • Assist patients in developing plans of action while providing necessary support and assistance
  • Support patients and caregivers in end of life care, including discussions around advanced care directives, palliative care options, placement to long-term care homes and other facilities
  • Maintain contact with other social service agencies and health care providers involved with patients to provide information and obtain feedback on progress
  • Provide counseling/therapy and other therapeutic services for individuals, families and groups as part of an interdisciplinary team
  • Practice within the scope and according to the standards of practice of the Ontario College of Social Workers as well as following established policies and procedures of the Family Health Team
  • Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the CFHT
  • Maintain standards of confidentiality and privacy, and
  • Other duties as required

Skills/Knowledge Requirements:

  • Knowledge and understanding of primary health care
  • An exceptional knowledge of the community’s resources
  • Proven ability to work in and contribute to an interdisciplinary health care environment
  • Exceptional interpersonal, oral and written communication skills
  • Complete documentation required to facilitate care for the patient (reports, referrals, forms, etc)
  • Ability to work independently and within a team
  • Ability to prioritize tasks, manage time and multi-task effectively in a very fast-paced work environment
  • High degree of computer proficiency
  • A valid driver’s license and car are required for home visits
  • Proven dependable attendance and performance record, and
  • Participate in self-directed learning to ensure his/her practice remains relevant (by attending professional conferences, e-learning and journal reviews)

Education/Specific Job Requirements:

  • Preferred completion of a Bachelor of Social Work or Masters of Social Work program
  • Registration with the Ontario College of Social Workers and Social Service Workers
  • Working knowledge of the Mental Health Act
  • Flexibility to work evenings if required
  • Police Record/Vulnerable Sector Check

Experience:

  • Three to five years minimum experience working within an inter-professional/collaborative team environment and preferably in a community-based setting
