Queen Square Family Health Team (QSFHT) provides comprehensive primary care to patients rostered to our Queen Square family physicians. Our programs include geriatric care, mental health care, post-hospital discharge review, diabetes care & palliative care amongst a wide range of other services and programs.  Our vision is to provide evidence-based, timely, integrated, interdisciplinary care that is of the highest quality.

In the recognition that mental illness and distress can lead to poorer health, shorter life expectancy, and poorer health-related quality of life, our Mental Health Team is in the process of developing a novel, evidence based, interdisciplinary, integrated model of primary mental health care for adults with a wide range of psychiatric disorders and mental health concerns.

Position Summary

 QSFHT is currently seeking a full-time registered social worker to work within our Mental Health Team.  This is a unique opportunity to contribute to the work of a novel and evidence based program.


Reporting to the Executive Director, and working within his/her full scope of practice, the Registered Social Worker will:

  • conduct comprehensive psychosocial assessments of adults with known / suspected mild to moderate psychiatric disorders
  • conduct collateral interviews with families / caregivers regarding psychosocial functioning as appropriate
  • conduct risk assessments pertaining to suicidal ideation/behaviour & /or self-harm
  • offer recommendations for clinical management and/or psychotherapeutic treatment planning
  • administer and score of psychometric measures for screening purposes under the clinical supervision of the psychologist when requested by the psychologist
  • contribute to the case formulation and development of the team’s integrated plan of care
  • liaise with community providers as needed to support implementation of clinical plan
  • provide evidence based psychotherapies – in particular individual and/or group Cognitive Behavioral Therapy & Dialectical Behaviour Therapy
  • provide clinical interventions designed to promote improved psychosocial functioning & symptom recovery
  • document clinical care activities in the patient’s EMR
  • contribute to program planning, implementation and evaluation of mental health program
  • collect data as required for statistical reporting purposes
  • participate in team building, program development and quality improvement activities
  • participate in team, program and FHT wide meetings
  • adhere to organizational policies
  • support and contribute to a healthy work place that embraces lifelong learning, diversity, clinical teamwork, privacy, and adherence to professional standards of practice
  • perform clinical duties as requested by the psychologist providing clinical supervision / mentorship
  • perform other duties as assigned by the Executive Director

Clinical / Professional Competencies & Requirements

  • MSW from a recognized program in clinical social work with a minimum of 2years experience working in a hospital-based or primary care clinical team providing service to adults with anxiety and/or mood disorders
  • registration with the College of Social Workers & Social Service Workers of Ontario in good standing & meeting legislative and College regulatory requirements to perform the controlled act of psychotherapy
  • proof of professional liability insurance
  • an undergraduate degree in Social Work or Psychology
  • clinical social work experience working with adults with mood disorders and anxiety disorders
  • additional experience with older adults and / or adults / seniors with medical disorders would be an asset
  • experience in the treatment of adults with psychiatric illness & a history of childhood trauma would be an asset
  • additional clinical experience working adults with mild ETOH and/or substance abuse would be beneficial
  • experience in conducting comprehensive psychosocial assessments
  • experience in conducting suicide risk assessments
  • experience in crisis intervention / management
  • experience in case formulation for psychotherapeutic treatment planning
  • experience in psychotherapeutic & counselling interventions to facilitate integration into the community and symptom / functional recovery
  • demonstrated knowledge, clinical skill & experience in the delivery of empirically supported individual and group psychotherapeutic modalities in the care of mild to moderate depressive disorders, anxiety disorders – in particular Cognitive Behavioral Therapy, Dialectical Behavior Therapy and problem focused and interpersonal approaches
  • demonstrated experience in therapeutic counselling of adults with respect to resource navigation and access to services / resources
  • demonstrated skills and interest in providing care within a collaborative interdisciplinary &/or shared care model of service delivery
  • working knowledge of community resources relevant to the provision of mental health care to adults with mild to moderate mental health concerns and /or adults/older adults with health concerns
  • experience in linking with community services and with Health Links as appropriate to ensure consistency of approaches and continuity of care
  • demonstrated working knowledge of the Mental Health Act, the Health Care Consent Act, & the Substitute Decisions Act
  • excellent oral and written communications skills
  • skills in critical thinking skills and clinical decision making
  • ability to work independently and collaboratively within an interdisciplinary collaborative / shared care model
  • demonstrated flexibility and ability to adapt to change
  • demonstrated experience and skills in use of computer technology for clinical care
