Registered Nurse – The Southlake Academic FHT (Newmarket)

Full-Time, 12 month contract

The Southlake Academic Family Health Team is seeking a Registered Nurse to join our Family Medicine Teaching Clinic working primarily out of our Newmarket location; however may also work in our Aurora, Bradford and Mt. Albert offices. The incumbent must be willing to work at least one evening per week and will provide comprehensive primary care services within the full RN scope of practice. These services will include core primary care, health promotion, disease prevention and chronic disease management as supported by the larger inter-disciplinary care team.

Duties:

  • Provision of direct primary care services for patients and families at all stages of growth and development
  • Assisting the team with program development to meet the needs of our patient population
  • Promoting of quality improvement initiatives which enhance care delivery processes and patient outcomes within the FHT
  • Supporting inter-disciplinary learners as they rotate through the clinic and related research activities
  • Practice within the context of our programs and services, including the provision of home visits.
  • Other duties as our clinic grows and evolves

Qualifications:

  • Current RN Certificate of Competence ( College of Nurses of Ontario)
  • Experience working in a primary care setting, preferably an academic setting
  • Demonstrated ability to work in a team environment which supports learners
