Registered Nurse (RN/RPN) Educator for Mental Health 

Full Time (1 position)

Rapids Family Health Team is seeking an enthusiastic, professional Registered Nurse (RN/RPN) educator with excellent communication and interpersonal skills to work collaboratively with other health care professionals to manage the needs of patients.  This is a mobile position; travel may be required between Rapids Family Health Team sites as well as throughout the community based on patient / community needs and business requirements.

As a Registered Nurse (RN/RPN) with mental health experience, you’ll leverage your nursing expertise and knowledge of community resources to assess patient needs, determine their eligibility for services and subsequently develop, evaluate and/or revise patient care plans.

Education and Qualifications:

  • Registered Nurse or Registered Practical Nurse with current registration in good standing with the College of Nurses of Ontario (preferably with Canadian Certification in Psychiatric/Mental Health Nursing)
  • Experience and detailed knowledge of mental illness, usually gained by several years of working with this population
  • Demonstrated knowledge and experience with crisis assessment and intervention techniques
  • Clinical experience working collaboratively within a multidisciplinary setting and with community mental health resources
  • Mental health and/or addictions, intake, triage and case management experience
  • Extremely well developed written and verbal communications skills
  • Demonstrated knowledge of and commitment to the principles of patient centred care
  • Planning and organization skills to facilitate advocacy and support coordination/service access
  • Efficient computer skills, working knowledge of electronic medical records
