POSITION: Registered Nurse – Diabetes Education Program (DEP)

  • Full-Time, Temporary (1 year maternity leave Oct 2018-Nov 2019)

REPORTING TO: Clinical Manager

 POSITION SUMMARY:                                                                                           

The Registered Nurse (RN) in collaboration with the DEP dietitian and pharmacist will provide primary care diabetes services to individuals and groups in accordance with acceptable clinical practice guidelines for diabetes.


  • Develop and implement diabetes education and interventions based on evidenced-based research, best practices, and current guidelines
  • Participate in community needs assessment, program development and evaluation
  • Lead and facilitate DEP individual and group sessions
  • Develop partnerships and engage in outreach initiatives
  • Participate in quality improvement initiatives
  • Acts as a coach and seek opportunities to strengthen the role of the Diabetes Nurse Educator within the DEP and the FHT
  • Participate in professional development and educational activities including providing clinical education to the multi-disciplinary team, students, and healthcare providers both within the FHT and externally
  • Participate in committees and research projects related to the DEP
  • Other duties as assigned


  • Baccalaureate degree in nursing from a recognized college or university
  • Registration with the College of Nurses of Ontario
  • Certified Diabetes Educator with the Canadian Diabetes Educator Certification Board required
  • Experience in diabetes nutritional assessment, management, education and counseling
  • Experience facilitating individual and group sessions utilizing adult education principles
  • Demonstrated planning and organizational skills
  • Ability to work independently as well as collaboratively within a multi-disciplinary team
  • Demonstrated ability to problem solve using sound judgement and critical thinking skills
  • Minimum 2 years experience working as a Diabetes Educator
  • Access to and use of a car is required
  • Occasional flexible hours may be required
