Registered Dietitian (Term) – North York FHT (Toronto)

POSITION:  Registered Dietitian
Full-Time Contract (February 2018-February 2019)
0.5 Dietitian & 0.5 Diabetes Education Program (DEP)

REPORTING TO:                Clinical Manager                                                                                         

POSITION SUMMARY:

The Registered Dietitian (RD) will provide specialized nutrition therapy in a primary care setting that is patient-centered and utilizes best practices to improve the health of the population. The RD will work collaboratively with the interdisciplinary family health team to ensure patients receive optimal evidence-based nutrition care.

RESPONSIBILITIES:

The RD will assess, record, monitor and evaluate patients, identifying priorities, strategies and interventions to create patients’ individualized nutrition treatment plan.  The RD will also coordinate and lead monthly group Diabetes Education Program (DEP) sessions and participate in DEP outreach initiatives. In addition, the RD will participate in professional development and educational activities, including providing clinical education to the multidisciplinary team, healthcare professionals and interns. Some evening hours will be required.

QUALIFICATIONS:

  • Bachelor’s degree in Food and Nutrition or equivalent degree with an accredited dietetic internship or equivalent supervised practical training required; post-graduate education preferred
  • A member in good standing with the College of Dietitians of Ontario.
  • Certified Diabetes Educator (CDE) with the Canadian Diabetes Educator Certification Board (CDECB) required or working toward this certification.
  • Demonstrated proficiency in Diabetes Education including nutritional assessment, management and counseling.
  • Demonstrated experience in assessing the nutritional status of patients, identifying concerns, developing care plans, and evaluating and monitoring the effectiveness of nutrition interventions for a variety of conditions such as: Diabetes, dyslipidemia, inflammatory bowel disease, celiac, IBS, pre/post-natal nutrition, pediatric selective eating, malnutrition etc.
  • Extensive experience managing chronic diseases through nutrition interventions.
  • Demonstrated experience in providing individual and group education.
  • Demonstrated ability to work independently as well as collaboratively within a multidisciplinary team.
  • Demonstrated ability to problem solve using sound judgment and critical thinking skills.
  • Strong organizational, interpersonal and communication skills.
  • Demonstrated computer literacy is required to facilitate the use of electronic medical records.
