Primary Care Nurse Practitioner (1.0 FTE)
6-Month Maternity Leave Contract

The Greenbelt Family Health Team is a dynamic group of physicians, nurses and other health care professionals. Our mission is to provide the residents of Barrhaven and the surrounding area access to high quality, comprehensive and timely medical care with the goal of promoting healthy families and a healthy community.

We are currently looking for a 1.0 FTE Nurse Practitioner for a 6-month maternity leave contract with the possibility of extension.

The Nurse Practitioner (NP) will function as a member of the primary care team in the provision of primary health care services.  The NP provides services to patients who include the individual throughout their life span, the family, and the community, with a strong emphasis on health promotion, patient education and disease prevention.

The NP is responsible for program appointments in the Hypertension, Women’s Health and Enhanced Primary Care programs.  Schedule includes daytime and evening shifts.


  1. Assessment
    • Working within the scope of practice s/he performs a comprehensive health or symptom directed assessment and synthesizes data from multiple sources to make a diagnosis, as part of the health care team
    • Works within medical directives as established and approved by the GFHT
    • Determines the need for, orders and interprets the results of screening and diagnostic laboratory and imaging tests within the scope of practice
  1. Treatment, Management and Planning
    • Provides advanced nursing care and treatment
    • Collaborates with physicians and other health care providers to develop and discuss appropriate care plan
    • Documents using patient health records, refers to and arranges follow-up as necessary
  1. Education
    • Help patients to identify and use health resources
    • Involve patients in decisions about their own health
    • Encourage patients to take action for their own health


  • Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program Certificate (or recognized equivalent)
  • Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred
  • Current registration with the College of Nurses of Ontario as an RN(EC) required
  • Current Membership with RNAO and evidence of liability insurance
  • Demonstrated experience in Community and/or Primary Care Setting
  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
  • Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills
  • Proficiency in the use of the computer hardware and software, particularly in Microsoft Word, Excel, Outlook
  • Prior experience with an electronic medical record
  • Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient
  • Excellent verbal and written communication skills
  • Effective written and oral communication skills
  • Demonstrated ability to work as part of a team
  • Bilingual preferred

Start Date: March 5 2018                                     Salary: $48 – $51 per hour
