Position:  Peer Advisor/Case Co-ordinator – NSM Trans Health Program (1 FTE)
(Trans* refers to all individuals who identify as transgender male or female, non-binary and/or gender non-conforming.)
Reports To:  Clinical Program Manager


Position Summary:

As a key member of the Couchiching Family Health Team (CFHT), and an integral part of an interdisciplinary health care team, the major role of the Peer Advisor/Case Co-ordinator will be to provide peer support, outreach and participate on the NSM Trans Health Committee as an advisor and person with knowledge or lived experience.  The Peer Support/Case Co-ordinator will facilitate services and support clients with their individual goals, utilizing their knowledge or experience with Trans health challenges. The individual will provide a source of encouragement and hope, partnering with clients as they pursue their objectives. The Peer Advisor/Case Co-ordinator will work collaboratively with clients as a coach and liaison, assisting clients to advocate for themselves. This individual will provide a bridge for Trans identified individuals in their community and will help to link clients with community resources of their choice. This role focuses on being a shared learner with clients, rather than being a helper, therapist or treatment provider, connecting with clients on an individual basis and also as a group facilitator.

Roles and Responsibilities:

  • Improve decision making processes, health outcome and client experiences for Trans-identified clients of all ages
  • Provide community-based short-term case co-ordination support to an assigned group of clients and assume responsibility for coordinating and/or facilitating client group sessions.
  • Work closely with clients to address problems, answer questions, gather and provide information, and connect clients to resources in the community
  • Collaborate with organizations and institutions on the development of policies and procedures that promote inclusion and belonging for Trans-identified individuals
  • Contribute to projects and activities that strengthen the Trans program and services
  • Administrative and operational duties including minute taking, emails, phone calls, documentation
  • Provide coverage to other administrative position during illness and vacation
  • Engage, establish and maintain relationships with clients
  • Promote independence through coaching and mentoring clients to set goals and develop skills
  • Use knowledge of Trans issues and/or lived experience to engage, support, encourage, and build hope with clients
  • Assist client to integrate into the community through accompaniment, planned activities and events
  • Planning, organizing, developing, leading and facilitating group activities, including education, and building skills and awareness of issues related to the Trans population
  • Contribute to program development, research and educational activities of the program
  • Ensure all notes and patient charts are updated in a timely manner
  • Prepare quarterly reports and monitor daily statistical data to ensure statistics are reported to the Ministry of Health in a timely fashion
  • Participate in CFHT and regional committees as needed
  • Provide education on Trans health-related issues to physicians and service providers
  • Maintain and upgrade knowledge and skills
  • Maintain current knowledge of policies and procedures
  • Maintain confidentiality in accordance with the Municipal Freedom of Information and Protection of Privacy Act
  • Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the CFHT

  • Excellent communication skills, including written and verbal presentation skills, diplomatic and effective liaison and interpersonal skills
  • Demonstrated ability to work effectively as a member of an Interprofessional team, with excellent interpersonal and communication skills
  • Exceptional documentation skills and attention to detail
  • Extensive knowledge of formal and informal community services and resources relevant to Trans population or demonstrated ability to search for applicable resources/information
  • Knowledge and understanding of the client population to effectively manage Trans barriers and challenges
  • Willingness to selectively share personal information that is relevant to a situation and for reasons that are therapeutic to the client
  • Has an appreciation for diversity
  • Possess good judgement and problem solving skills
  • Excellent time management skills with an ability to multi-task and effectively set priorities
  • Effective leadership, decision-making, organizational and problem solving skills are required
  • Ability to demonstrate a non-homophobic and anti-racism approach
  • Ability to demonstrate an anti-oppression approach to practice


Education/Specific Job Requirements:

  • Formal education in a relevant field of study (Social Science, health promotion, social work, adult education etc.) or the equivalent combination of education and work experience
  • Registration with appropriate professional College if applicable
  • Familiar with the World Professional Association for Transgender Health (WPATH) Standards of Care
  • Familiar with the Guidelines and Protocols for Hormone Therapy and Primary Health Care for Trans Clients (Sherbourne Health Centre)
  • Proof of current Ontario Driver’s License and access to reliable vehicle for travel to Midland, Barrie, Orillia, Muskoka, and Collingwood
  • Provide evidence of a clean driver abstract and proof of insurance
  • Able to work flexible hours including evenings if required
  • Police Record/Vulnerable Record Check



  • At least 2 to 3 years in Trans Health services, peer support, or relevant work experience
  • Experience working with clients, staff, and organizations of diverse backgrounds are required
  • Experience facilitating groups
  • Experience with an Electronic Medical Record (EMR) and office applications
