Occupational Therapist (PT, Term) – The Centre for Family Medicine FHT (Kitchener)

The Centre for Family Medicine FHT is currently seeking a

Part-Time Occupational Therapist

to join our family health team to cover a maternity leave. Our mission is To provide leadership and excellence in comprehensive patient-centred primary care through education, research, innovation and care provision in a collaborative, sustainable, inter-professional environment.

Our team currently consists of twenty family physicians, nurse practitioners, nurse educators, registered practical nurses, registered nurses, part time specialist physicians, social workers, family and marriage therapists, dietitians, chiropodists, occupational therapist and clinical pharmacists.  We are associated with optometry and a chiropractor. The Centre for Family Medicine’s academic Family Health Team is comprised of three sites and is an integral part of the University of Waterloo Integrated Health Building. We work in a fully computerized environment and encourage continued educational experiences.

The Occupational Therapist will work with a variety of clinics within the Centre for Family Medicine based on specific populations including but not limited to the Memory Clinic and the Mobility Clinic. The occupational therapist will also see patients based on direct referral from the FHT physicians. Responsibilities will include assessments and intervention related to falls and balance, prescription of assistive devices including walkers and wheelchairs, cognitive impairment, home safety, functional impairment and system navigation. The occupational therapist will work in both the clinic environment and the community providing home visits and interventions within patients home.

Qualifications and Experience:

  • Minimum of 3 years relevant experience and knowledge in several areas of Occupational Therapy including: dementia, geriatrics, mobility impairments, falls, home safety, seating and mobility, chronic disease and mental health.
  • Current registration with the College of Occupational Therapists of Ontario
    • Ontario Ministry of Health and Long-Term Care Assistive Devices Program (ADP) Authorizer status for
      ambulation aids and wheelchairs is preferred
    • Experience in community based care and knowledge of local health and social resources is an asset
    • Demonstrated ability to work within scope of practice
  • Demonstrated ability to work collaboratively with physicians, interdisciplinary healthcare providers and
    other community partners
  • Demonstrated clinical reasoning skills and excellent assessment and therapeutic skills
  • Interest in participating in research and desire to work in a continuous learning environment
  • Valid driver’s license, access to a vehicle and willingness to travel to patients’ homes within the Kitchener-
    Waterloo area (urban and rural)
  • Ability to work independently without direct occupational therapy supervision
  • Vulnerable Sector Check required
