Salary Range:                    New Graduate – $92,000; After 6 months – $94,048

The Nurse Practitioner (NP) will work autonomously and collaboratively in providing individual patient care assessment and treatment in core clinical programs, including complex and chronic disease management, mental health, cancer prevention and palliative care. The NP within the Scarborough Academic Family Health Team (SAFHT) will provide consulting and supporting services for patients in their own practices and those referred by team family physicians and other team clinicians. In this regard the NP will act as a strong resource to all the Interdisciplinary Health Providers (IHPs) and family physicians within SAFHT.

The two NPs within SAFHT share the lead for the Chronic Disease Management Team and the Seniors Home Visit Team.  They provide teaching and resources to the RNs regarding CDM programs, care for seniors and shared palliative care.

As a senior clinician, it is anticipated that the NPs will work closely with the Executive Director and other staff to build strategic programs from delivery through to evaluation.  The NPs within SAFHT in part will focus their activities on supporting the enrollment of currently unattached and unrostered patients within the eastern Toronto community.


The Nurse Practitioner reports to the Executive Director of the Scarborough Academic Family Health Team (SAFHT) or the delegate in case the Executive Director is unavailable. The Nurse Practitioner is to practice within the scope and according to the standards of practice for Nurse Practitioners as outlined in the following documents:

  • College of Nurses of Ontario’s “Practice Standards” (
  • Nursing Act (1991) and
  • Regulated Health Professions Act (1991)


  • Registered Nurse in the extended class with a certificate issued by the College of Nurses of Ontario.
  • Nurse Practitioner certificate in Primary Care.
  • Baccalaureate of Science in Nursing is required, Master’s degree preferred.
  • Current BCLS certification.
  • Member of RNAO required, NPAO preferred.


  • Experience in a family health team is preferred.
  • Experience in the areas of chronic disease management, seniors care, mental health and the provision of cancer care screening and palliative care is preferred.
  • Independent health care assessment and patient management skills are required.
  • Experience working with interprofessional health care teams and providing collaborative support within that setting.
  • Demonstrated experience in ethno-culturally diverse environments is preferred.
  • Demonstrated ability to prioritize and work effectively in a fast paced environment.
  • Demonstrated ability to participate in professional development implementations and program evaluation is preferred.
  • Demonstrated commitment to continuous learning and innovation.
  • Strong written and verbal interpersonal and communication skills are required.
  • Computer skills are required (Electronic Medical Record, Microsoft Office).
