The Nurse Practitioner (NP) will work autonomously and collaboratively in providing individual patient care assessment and treatment in core clinical programs, including complex and chronic disease management, mental health, cancer prevention and palliative care. The NP within the Scarborough Academic Family Health Team (SAFHT) will provide care, consulting and supporting services for patients in their own practices and those referred by team family physicians, unaffiliated physicians and other team clinicians. In this regard the NP will act as a strong resource to all the Interdisciplinary Health Providers (IHPs) and family physicians within SAFHT.

Two full time and two part-time NPs within SAFHT share the clinical lead for the Chronic Disease Management Program Teams and the Complex, Vulnerable (FOCUS) Program as well as the Healthy Aging Program.  They provide teaching and resources to the RNs regarding CDM programs, care for seniors and shared palliative care. They share care where appropriate with the physician assistant in the Healthy Aging Program.

Since April 1, 2018 SAFHT is required to adopt a wide-population health planning approach in its delivery, levering partner resource availability where appropriate. A core element is bringing Team Based Care to local practices in the three LHINs where SAFHT primary operates. This necessitates an active involvement in raising the consciousness of local physicians to the team based approach.  Additionally the implied program development aspect is part of the NP role with the program manager in SAFHT.

As a senior clinician, it is anticipated that the NPs will work closely with the Executive Director and other staff to build strategic approaches to programs from delivery through to evaluation.  The SAFHT NPs may spend time enrolling currently unattached and unrostered patients within eastern Toronto.


The Nurse Practitioner reports to the Executive Director of the Scarborough Academic Family Health Team (SAFHT) or the delegate in case the Executive Director is unavailable. The Nurse Practitioner is to practice within the scope and according to the standards of practice for Nurse Practitioners as outlined in the following documents:

College of Nurses of Ontario’s “Practice Standards” (

Nursing Act (1991) and

Regulated Health Professions Act (1991)


Registered Nurse in the extended class with a certificate issued by the College of Nurses of Ontario.

Nurse Practitioner certificate in Primary Care.

Baccalaureate of Science in Nursing is required, Master’s degree preferred.

Current BCLS certification.

Member of RNAO required, NPAO preferred.


  • Experience in a family health team is preferred.
  • Experience in working with multiple care teams in diverse settings is valued.
  • Experience in the areas of chronic disease management, seniors care, mental health and the provision of cancer care screening and palliative care is preferred.
  • Independent health care assessment and patient management skills are required.
  • Experience working with inter-professional health care teams and providing full scope collaborative support within that setting.
  • Demonstrated experience in ethno-culturally diverse environments is preferred.
  • Demonstrated ability to prioritize and work effectively in a fast paced environment.
  • Demonstrated ability to participate in program development, implementation and evaluation is preferred.
  • Demonstrated commitment to continuous learning and innovation.
  • Strong written, verbal interpersonal and communication skills are required.
  • Computer skills are required (Telus – Electronic Medical Record, Microsoft Office).
