Permanent Part Time – Nurse Practitioner

The Niagara North Family Health team is looking for a creative, innovative and highly motivated Nurse Practitioner to work in collaboration with our physicians, staff and inter-disciplinary health professionals at our Niagara-on-the-Lake (Virgil) site. This position is a permanent, part-time position (37.5 hours biweekly). The Nurse Practitioner will provide full-scope primary care to patients with a wide variety of health conditions as well as supporting the various programs of the FHT.

The primary responsibility of the NP is to provide comprehensive direct patient care as a central member of the team, with a focus on:

  1. Health Promotion and disease prevention
  2. Management of chronic illnesses
  3. Management of acute episodic illnesses or injuries.

The NP works collaboratively with all members of the Family Health Team and practices in accordance with the College of Nurses of Ontario Practice Standard: Nurse Practitioners. The NP supports excellence in patient care services through on-going professional education, advance knowledge through research and/or participation in scholarly activities.


  • Graduate of the Ontario Primary Heath Care Nurse Practitioner Program (or recognized equivalent)
  • Baccalaureate of Science in Nursing Required, Master of Science in Nursing preferred
  • Current Registration with the College of Nurses of Ontario as an NP-PHC
  • Evidence of liability insurance
  • Demonstrated experience in community and/or primary care setting
  • Knowledge and proficiency in current, evidence based methods and practices of primary health care delivery, with an emphasis on health promotion.
  • Superior leadership, organization, research, evaluation, time management, communication and interpersonal skills
  • Proficiency in the use of technology including electronic health records and common software applications such as Word, Excel and Power Point
  • Desire and ability to update knowledge and skills through various means including technology based opportunities, courses, workshops and
  • Ability to work independently and cooperatively in a busy multidisciplinary team
