Nurse Practitioner – Kapuskasing and Area FHT (Kapuskasing)

Nurse Practitioner

The Kapuskasing and Area Family Health Team is a collaborative interdisciplinary team of physicians and primary health care providers.  We are currently seeking a full-time motivated and organized Nurse Practitioner to join our team.  As a key member of the Family Health Team, the Nurse Practitioner will work within the full scope of primary health care nurse practitioner practices.

Responsibilities

  • Participates in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client population
  • Determine the need for, order, perform and review appropriate diagnostic tests
  • Provide patients with appropriate and understandable clinical advice, education and counselling, based on sound and up-to-date medical knowledge
  • Accurately document in the patient record all assessments, observations, conversations, interventions and clinical discussions relating to patient care
  • Collaborate and liaise positively with physicians, office staff and other team members to optimize outcomes to the patient
  • Arrange for referrals (internal and external) as necessary
  • Participate in case management discussion
  • Participate in team activities and attend team meetings
  • Participate in evaluation of the quality and effectiveness of practice, to contribute to positive changes in programs and care

Qualifications/Skills

  • Masters of Nursing
  • Baccalaureate Degree in Nursing, and successful completion of the Ontario Primary HealthCare Nurse Practitioner Certificate Program
  • Current registration with the College of Nurses of Ontario as an RN(EC) required
  • Current Membership with RNAO or NPAO and evidence of liability insurance
  • 2 years of experience in Community and/or Primary Care Setting
  • Knowledge and proficiency in current evidence-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
  • Ability to demonstrate customer focused skills which align with our Vision, Mission and Values
  • Fluency in English and French will be an asset
