Mental Health Nurse – Burlington FHT (Burlington)

Position:              Mental Health Nurse 1 FTE
Reporting to:      Executive Director & Physicians (for clinical responsibilities)

Position Summary:

The Mental Health Nurse (MHN) is mandated to co-ordinate access to mental health services, group programs as well as individualized support for patients of the Burlington Family Health Team (BFHT). The MHN works collaboratively with the Mental Health Team to triage incoming referrals in order to link them to appropriate internal and external supports and resources. Furthermore, the MHN plans, implements and evaluates new program opportunities to reflect the needs of the patients of the BFHT as well as to fill gaps in services.

Responsibilities:

Primary Role

  • Triage incoming referrals from FHT physicians or Interdisciplinary Health Providers (IHP) and link these patients with the appropriate internal or external resources
  • Provides support to patients with chronic mental illness. This could include fact to face or telephone support, applications for financial support and medication management
  • Will lead or collaborate with other health care providers for provision of group classes and specialized program delivery. This includes planning, implementation and evaluation
  • To develop, based on patient and community need, additional programs using program planning and evaluation framework
  • Links patients with appropriate community supports, health care practitioners or directs patient to BFHT group classes or individual counseling
  • Support patients who wish to quit smoking through our Smoking Cessation Program in association with CAMH and the Ottawa Model for Smoking Cessation

Qualifications

  • Current registration with the College of Nurses of Ontario
  • Mental Health Experience including counselling
  • Experiencing using cognitive behavioural therapy
  • Understanding of patient privacy and confidentiality
  • Strong knowledge of community resources in mental health
  • Skills in program planning and development
  • Current CAMH TEACH certificate is an asset

Education

  • Bachelors of Science in Nursing (BscN)

Experience

  • Minimum of 3 years working in a Family Health Team Setting
  • Experience in developing and maintaining Quality Improvement Programs.
