JOB POSTING: EXECUTIVE DIRECTOR, CREDIT VALLEY FAMILY HEALTH TEAM
The Credit Valley Family Health Team Board of Directors is seeking a demonstrated dynamic and innovative health care leader to serve in the position of Executive Director. The Credit Valley Family Health Team is an Academic Family Health Team affiliated with Trillium Health Partners and the University of Toronto. It is an engaging multi-disciplinary primary care team dedicated to continuous learning and excellence within primary care with a demonstrated track record of impactful quality improvement activities. The Executive Director role also has an exciting growth opportunity, as the team has recently received $1.3M additional base funds to expand the Interprofessional Care Team (IPCT) model into the North West Mississauga Care Community.
The successful candidate will be an experienced and seasoned health care leader with demonstrated excellence in executing in a lean organization. They will be working closely with the Board of Directors in setting the future direction of the organization through the upcoming cycle of strategic planning. The successful candidate will have a clear vision on how to articulate the strategic plan into deliverable key activities. They will be experienced in program development, measurement, and quality improvement methodologies. They will demonstrate in their previous work impeccable integrity and excellent communication skills with employees, stakeholders and community members.
