Diabetes Nurse Educator Recruitment (0.6 FTE):

Located in northern Huron County in Southwestern Ontario, the North Huron Family Health Team is a progressive team of health professionals with an integrated approach to healthcare.   Our office is located in Wingham with clinics in Lucknow and Teeswater.  We are currently seeking a Diabetes Nurse Educator with strong commitment and the ability to work collaboratively in a team setting.

Position Summary:

This position will support the Diabetes Education Program for the North Huron Family Health Team and Huron-Perth Diabetes Education Program. The office is located in Wingham.  As a member of the Diabetes Education Team, the Diabetes Nurse Educator (DNE) plans, develops, implements and evaluates diabetes education services.  The DNE will provide direct patient care making recommendations regarding care and treatment, advocating for adults with diabetes and facilitating communication between the patient, family, physician, specialist and other health care professionals. Through health promotion activities, the DNE will increase the awareness of diabetes and associated risk factors within the community at large.  The DNE acts as a professional resource for health care providers in the area.

Qualifications:

Qualifications for this position will include:

  • Current Registration with the College of Nurses of Ontario, with a Registered Nurse designation

Certified Diabetes Educator (CDE), or in process

  • Minimum 1 year experience in diabetes education
  • Experience in group facilitation and knowledge of adult education methods
  • Effective teaching skills for individuals and groups
  • Experience with insulin education and glucose pattern management
  • Demonstrated excellent interpersonal, verbal and written communication skills
  • Demonstrated counseling, problem-solving and assessment skills
  • Demonstrated application of chronic disease self-management supports
  • Self-directed, motivated team player with a commitment to ongoing professional development
  • Awareness of community resources for the person/family with diabetes
  • Knowledge of current standards and guidelines for the management and education of people with diabetes
  • Computer competency using MS Word, Outlook, Excel

SALARY: $34.54 to $36.78 per hour
