Community Outreach Nurse, Part Time  Maternity Leave  

Niagara North Family Health Team, Lake-Carlton (St. Catharines) Site

The Niagara North Family Health Team is an integrated, collaborative, teaching family health team providing excellent care to patients in the Niagara region.  Our vision is to work with our patients and families towards optimizing their health status and well-being. Our focus is on health care, health promotion, disease prevention and chronic disease management.


The Lake Carlton Site of the Niagara North Family Health team is looking for a creative, innovative and highly motivated Part Time Registered Nurse (20 hours per week) to cover a maternity leave with an interest in the management of chronic disease to work in this permanent part time position within our programs and services. Experience or interest in aging populations is an important part of this position. In addition, this position will involve the development of partnerships with various community agencies and our local hospital to facilitate patient transitions in order to manage the care of complex patients. The successful candidate will work collaboratively with all ongoing programs, interdisciplinary health professionals as well as our primary care physicians.


Registered Nurse with experience in the delivery of health services in the community.

Current Registration with the RNAO – must provide evidence of personal liability insurance

Excellent communication and interpersonal skills

Ability to problem solve and think critically

Proficient in the use of computer and information technology, experience with electronic health records an asset.

Interest in the development and management of programs aimed at promoting health and wellness

Experience in the development of partnerships within a community setting

Ability to work independently and cooperatively in a busy multidisciplinary team
